Terms and Conditions
     Services
      ·10 minute consultations
      ·Telephone consultations
      ·Home visits
      ·Repeat prescriptions
      ·Clinical administration
      ·QOF literate and pro-active in ensuring up-to-date
      ·Minor Surgery (subject to negotiation)
      ·Nexplanon fitting / removal (subject to negotiation)
· Trained in telephone triage
      ·EMIS & Synergy literate

     Booking arrangements
     ·Bookings should be confirmed in writing (email or letter) with a clear outline of the job requirements.
     ·Once a contract is signed for the relevant period of work, all future bookings with the practice will be subject to the
     same 'Terms and Conditions' without the need for a further contract to be signed.

     Typical sessional periods and intervals
     ·Consultation time should be a minimum of 10 minutes per consultation.
     ·A typical ‘half day’ session is no longer than 4 hours. This includes 2.5 hours consultingtime* and 0.5 hours for
     administration.
     *Consulting time refers to pre-booked consultations, add-ons and telephone consultations.
     ·A typical 2 hour surgery session will therefore consist of a maximum of 12 consultations, with any additional cover
     being charged at the hourly rate (charged in half hour increments).
     ·A typical ‘full day’ booking consists of two ‘half day’ sessions as outlined above. The midday period will be free from
     practice commitments, unless separately negotiated Session requirements are flexible but alterations must be agreed
     in writing at time of booking.

     Additional consultations and tasks
     ·I do not charge for small overruns, providing the workload is reasonable and adheres to the guidance outlined above.
     ·Any significant overrun or work outside of agreed sessions (due to add-ons, telephone consultations, home visits,
     administration etc.) will be charged at the agreed hourly rate, in half hour increments.

     Signing prescriptions
     ·For medico-legal reasons, I can only sign repeat prescriptions and not acute prescriptions generated by others.
     ·I am willing to consult patients with drug or alcohol issues, however, I cannot sign prescriptions for maintenance or
     withdrawal treatment (opiates, benzodiazepines etc.).

     Visits and On-Call
     ·Visits (if requested) will only be performed in morning sessions (unless covering on-call).
     ·On-call or duty doctor cover must be agreed at the time of booking and is subject to an additional charge.

     Drugs and equipment
     ·I will provide my own basic medical equipment.
     ·The practice must supply emergency equipment and drugs (e.g. nebulisers, IM drugs, defibrillator etc.).

     Practice information
     ·I would prefer a current practice information pack to help with my induction.
     ·If the practice uses a computer system that I am not familiar with, I will require some brief training prior to the first
     session.
Certified end-of-placement report
       ·Due to GMC revalidation, I may ask the practice to provide an end-of-placement report.

       Mileage and parking
       ·Mileage will be charged at 70p/mile for return travel and visits (capped at £30.00).
       ·The practice will cover any additional costs (parking, tolls etc.) of travel.

       Private/non-NHS work
       ·Private work (e.g. medicals, reports and certificates) would not normally be undertaken unless in exceptional
       circumstances and if agreed at time of booking.
       ·This will be charged at the practice rate.

       Cancellation
       ·I reserve the right to cancel a booking if any of the terms and conditions are breached.
       ·I will give 28 days notice if I am unable to honour a booking, unless unforeseen circumstances arise (e.g. illness).
       ·Cancellation by the practice on the day of the session will incur a 100% charge.
       ·Cancellation by the practice within 7 days of the session will incur a 75% charge.
       ·Cancellation between 8 and 21 days of the session will incur a 25% charge.

       Payment
       ·The practice will be invoiced at the end of each booking or fortnightly whichever is sooner.
       ·Late payment after 14 days of invoice issue (including bank holidays) will be subject to a 10% weekly-rising surcharge.
       ·Payment by BACS or cheque is acceptable.
       ·A signed superannuation form (NHS Pension Form A) will be required for all sessions.

Cancellation ·I reserve the right to cancel a booking if any of the terms and conditions are breached. ·I will give 28 days notice if I am unable to honour a booking, unless unforeseen circumstances arise (e.g. illness). ·Cancellation by the practice on the day of the session will incur a 100% charge. ·Cancellation by the practice within 7 days of the session will incur a 75% charge. ·Cancellation between 8 and 21 days of the session will incur a 25% charge.
