The document is a student evaluation form for a service learning placement through the GBS Year Three programme. It asks the student to rate their agreement with 11 statements about various aspects of the pre-placement information, academic support during placement, assessment methods, and support from supervisors and staff at the placement site. Space is also provided for the student to elaborate on any areas of disagreement and to provide any other important feedback.
