The New Horizons PATH team works to help homeless individuals with mental illness in Columbus, Georgia. On any given night, over 1,500 people in Muscogee and Russell counties lack permanent housing, and many homeless individuals struggle with disabilities or substance abuse issues. The PATH team connects homeless people to housing, healthcare, benefits assistance, and other services to help the city meet its goal of ending homelessness. They partner with groups like the Columbus Regional Mobile Health Unit and help clients obtain housing vouchers. One client, Dale Chaffin, was able to move into his own apartment with a voucher and apply for SSI benefits with PATH's help.

Increasing income through employment is a key component in ending homelessness. This workshop will focus on new initiatives to expand employment opportunities for low income individuals and families, including models such as subsidized and transitional jobs. TANF Emergency Contingency Fund and other funding strategies to support these initiatives will be discussed.

This workshop will present strategies to increase employment income for families and individuals who are served in HPRP and other rapid re-housing programs. HPRP coordinators and providers should attend if they are looking for ways to rapidly connect program participants with jobs and help them increase their incomes

According to Home For Good of United Way of the Chattahoochee Valley, on any
given night in Muscogee and Russell Counties, more than 1,500 people lack perma-
nent housing. In addition, 47% of the homeless population in Columbus has a disabil-
ity, including a mental illness, and 37% report using alcohol or drugs. For many, the
path to homelessness is triggered by unemployment and financial issues. The City of
Columbus has enacted a ten-year plan to end homelessness by making it “atypical,
temporary and non-recurring” and is actively participating in Zero: 2016, a cam-
paign to functionally end chronic and veterans’ homelessness by December 2016.
The New Horizons PATH Team works hard to help the city reach its goal of ending
homelessness in our community by working to house mentally ill homeless individuals
and get them mental health treatment.

The Columbus Regional Mobile Health Unit is a
community outreach program of the Columbus
Regional Health system. The Mobile Health
Unit brings physicians, pharmacists and nurses
to the public to provide general wellness, dia-
betes and blood pressure screenings via a mo-
bile bus equipped with two screening rooms
and testing equipment. The Mobile Health Unit
visits local schools, businesses, and shelters to
promote health education and awareness. For many homeless and low-income individuals in the Columbus ar-
ea, the Mobile Health Unit is a primary source of healthcare and a vital link to hospitals, physicians and treat-
ment.
The New Horizons PATH Team conducts outreach with the Mobile Health Unit twice a month at SafeHouse and
Valley Rescue Mission. The Mobile Health Unit goes into the community where they are needed most to pro-
vide critical services. We are honored to be able to partner with such a great organization that provides much
-needed healthcare to the neediest people in our community!
Community Collaborations: Georgia Housing Voucher Program

The Georgia Housing Voucher is a program administered by the
Georgia Department of Behavioral Health and Developmental Dis-
abilities (DBHDD). This program provides supportive housing to
homeless individuals with mental illness., primarily by assisting
voucher recipients with security and utility deposits, rent and living
expenses, and furniture and groceries. Through the Georgia Hous-
ing Voucher, the State of Georgia hopes to house 2,000 homeless
individuals with severe and persistent mental illness by the end of
2015.
New Horizons Behavioral Health and the New Horizons PATH Team
have stepped up their efforts to house the most vulnerable of the
local population. Each PATH Team member helps individuals apply
and interview for the Voucher and then helps them locate perma-
nent housing. New Horizons is grateful to be a service provider for
the program and hopes to house many more people find stable and
supportive housing in the year to come!

PATH News: PATH Goes to SOAR Training
PATH to Success: Dale Chaffin
On May 20-21, the New Horizons PATH Team attended
training to become certified in SSI/SSDI Outreach, Access
and Recovery (SOAR). The aim of SOAR is to help end
homelessness by increasing access to federal benefits, em-
bodying the idea that “To recover, people need a safe
stable place to live.”
The PATH Team learned about the process of helping
homeless individuals apply for SSI and SSDI benefits to
get people approved on the first application. Each mem-
ber of the PATH Team is now capable of assisting home-
less individuals in applying for federal benefits and has
learned the proper steps to get more people benefits.
We want to thank the trainers for providing us with the
opportunity to give our individuals access to benefits and
increasing their self-sufficiency. For more information

When Dale Chaffin first encountered the New Horizons PATH Pro-
gram in December 2014, he was living at the Salvation Army. In
March 2015, he had moved into his own apartment subsidized by the
Georgia Housing Voucher Program, thanks to help provided by the
PATH Team.
Dale applied and was approved for a Georgia Housing Voucher,
which assists him with rent, utilities and furniture in his own apartment.
Dale’s PATH case manager helped him apply for SSI benefits to help
him develop self-sufficiency. He is now happily living on his own and
spending time with his family, especially his six-month-old grand-
daughter.
Dale is just one of the many people who has been helped by the New
Horizons PATH Team and the Georgia Housing Voucher Program. We
are happy that he has found permanent housing and wish him the
best of luck as he continues his recovery! Dale sitting in his apartment

The Projects for Assistance in Transition from Homelessness (PATH)
program provides services to people with serious mental illness,
including those with co-occurring substance use disorders, who are
experiencing homelessness or at imminent risk of becoming home-
less.
There are nearly 600 local organizations that provide PATH ser-
vices. Local PATH programs offer services including outreach, ha-
bilitation programs, community mental health, alcohol and drug
treatment, and case management services.
The New Horizons Behavioral Health PATH Program of Columbus,
Georgia desires to see every individual in our city under a roof in
a warm bed at night. The New Horizons PATH Program seeks to
collaborate with area landlords and employers to bridge the gap
between housing and the homeless and between employment and
the unemployed.
It is our hope through our quarterly newsletter that we share with
the community stories of hope, challenges and triumph. We want
to share the stories of those who are in the trenches with us to help
fight against homelessness and poverty.
