This book has been designed to give you the ability to write down
all of your medications, a twelve-month calendar to keep up with future
doctor appointments, and a place you can write down your blood sugar
levels, blood pressure levels, weight, and much more.
My name is Angela Laws. I have worked in a hospital setting for over
twenty years. I am a bachelor’s level social worker and currently completing
my master’s level of social work at the University of South Carolina. I have
two years’ experience working as a care transition coach, where the idea for
this book came from. During my time as a care transition coach, I learned
that many of my patients struggled with understanding their medications,
remembering doctor’s appointments, and keeping up with all the medical
information they were asked to keep up with by their doctors. My passion
for people inspired me to create something that would help people manage
so much information in one place. I know what it is like having to keep up
with tons of information with being a mother of three children, a college
student, an employee, and a wife.
