This document discusses improving the effectiveness of psychotherapy, especially for challenging cases. It provides both good and bad news about therapy outcomes. On average, treated clients do better than 80% of untreated clients. However, dropout rates are around 47% and some therapists fail to identify clients who are not progressing. The document recommends formalizing client feedback through measures of outcome and alliance administered at each session.
