This document is a six month notice form for a customer to transfer their gas and electric service agreements from Pacific Gas and Electric Company (PG&E) to direct access service. By submitting this form, the customer is providing six months notice of their intent to transfer to an Energy Service Provider (ESP). PG&E will provide confirmation of the deadline by which the ESP must submit a Direct Access Service Request (DASR) to complete the transfer. If the DASR is not received by that deadline, the customer's service will be switched to a transitional bundled service for up to 60 days before returning to bundled portfolio service with a new commitment period.

1. Six Month Notice To Transfer To Direct Access Service
This completed form notifies Pacific Gas and Electric Company (PG&E) of your intent to transfer your
service agreement(s) to direct access (DA) service. You will receive confirmation from PG&E within ten
business days that will specify the date your Energy Service Provider (ESP) must submit a Direct Access
Service Request (DASR) to PG&E in order to complete the transfer of your service agreement(s) to DA
service. This is important information that you will need to provide to your ESP to complete your request.
This date is significant because if PG&E does not receive a DASR by this date, your service agreement(s)
will be switched to Transitional Bundled Service (TBS) for a period of up to 60 days. If a DASR is not
received by the end of this 60 day period, then your six month notice to return to DA service will be
cancelled, and the cancellation will serve as your six month notice to return to bundled portfolio service.
You will remain on TBS for a period of six months and then begin a new three year commitment period on
bundled portfolio service.

Consider this my six month notice to transfer the service agreement(s) listed below to DA service. I
understand that my Energy Service Provider must submit a Direct Access Service Request (DASR) on
my behalf prior to the specified date in order to complete the transfer. I understand the rules and
conditions as set forth in PG&E’s Rule 22.1 pursuant to California Public Utilities Commission
(CPUC) Decision 03-05-034 and CPUC Resolution E-4006. Once received by PG&E, this Six Month
Notice To Transfer To Direct Access Service form may be rescinded within three business days by
contacting PG&E at (800) PGE-5000. I understand that after the three business days have elapsed, this
notice is irrevocable.
* Please note that customers on Demand Response programs Critical Peak Pricing (CPP), Scheduled
Load Reduction Program (SLRP) and Base Interruptible Program (BIP) are not eligible for these
programs under DA service.
