This document presents a case report of necrotizing sialometaplasia (NS) in the minor salivary glands of the hard palate in an HIV positive patient. NS is a rare benign inflammatory disease of the minor salivary glands. The patient presented with an irregular ulcer measuring 2.5x1 cm on the hard palate. Histopathological examination revealed features characteristic of NS including pseudometaplasia, lobular necrosis, and chronic inflammation. The lesion disappeared completely after 2 weeks without further treatment. NS is an important diagnosis for clinicians to be aware of, especially in immunocompromised patients, as it can be mistaken for cancer but is actually a self-limiting condition
