4. DEFINITION
o ESWT or Shockwave therapy is a noninvasive method that uses acoustic
waves to treat various musculoskeletal condition.
o Shockwave = intense short energy wave travelling faster than speed of
sound.
o Well controlled mechanical insult to tissue.
o Fast pain reliever and restore mobility.

HISTORY
o In 1983 the first commercial lithotripter, the Dornier Lithotripter
o Since 1985 other organs such as the gall bladder, bile duct,
pancreas and salivary glands have been treated with shockwave.
o During the next 10 years shockwave machines were trialed and
have been used to treat orthopedic disorders, including
osteochondrosis and various tendinopathies (Haupt, 1997).

6. SHOCKWAVE THERAPY
FAST AND PERMANENT RELIEF FROM PAIN
o Unique, non-invasive solution for musculoskeletal pain
o Just three to four treatments needed at weekly intervals
o A therapy session only takes about 10 minutes
FIELDS OF APPLLIACTION
o Orthopaedics
o Rehabilitation
o Sports medicine

7. MECHANISM OF ACTION
o A shockwave is an acoustic wave which carries high energy to
painful spots and myoskeletal tissues with subacute, subchronic
and chronic conditions. The energy promotes regenerating and
reparative processes of the bones, tendons and other soft tissues.
7
Shockwaves are characterized by jump change in
pressure, high amplitude and non periodicity.
The KE of the projectile created by compressed air is
transferred to the transmitter at the end of the
applicator
9. MEDICAL EFFECTS
o Acoustic waves with high energy peak used in Shockwave therapy
interact with tissue causing overall medical effects of accelerated
tissue repair and cell growth, analgesia and mobility restoration.
o All the processes mentioned in this section are typically employed
simultaneously and are used to treat chronic, sub-acute and acute
(advanced users only) conditions.
o New Blood Vessel Formation
o Reversal of Chronic Inflammation
o Stimulation of Collagen Production

10. CONTINUED…
o New Blood Vessel Formation:
Nutrient blood flow is necessary to start and maintain the repair processes of
damaged tissue. The application of acoustic waves creates capillary micro
ruptures in tendon and bone.
o Reversal of Chronic Inflammation:
Mast cells are one of the key components of the inflammatory process. Their
activity may be increased by using pervasive acoustic waves.
o Stimulation of Collagen Production:
The production of a sufficient amount of collagen is a necessary precondition
for the repair processes of the damaged myoskeletal and ligamentous
structures, Shockwave therapy accelerates procollagen synthesis.

o Non-invasive solution for chronic pain in your shoulder, back,
heel, knee or elbow
o No anaesthesia required, no drugs
o Limited side effects
o Main fields of application: orthopedics, rehabilitation, and sport
medicine
o New research shows that it can have a positive affect on acute
pain

15. HOW SUCCESSFUL IS
SHOCKWAVE?
Shockwave therapy is one of the most widely researched rehabilitation
modalities used in physiotherapy clinics with increasing numbers of scientific
studies added each year.
The majority of research validates the effectiveness of extracorporeal
shockwave, and shows successful treatment rate of up to 90%
