Student presents academic
and/or behavior issues.
Complete Tier I Intervention Plan
Contact student’s parents to
discuss Tier I including concerns
and interventions.
If interventions are
successful…
Continue with interventions.
Prior to a Tier III referral, the teacher should develop
a Tier I, make at least two parent contacts, complete
Tier II, meet with grade level PLC to discuss the
student, implement interventions for at least 4-6
weeks. Be sure all forms are complete and
necessary data and documentation have been
collected before referring the student to the Student
Assistant Team (SAT). All forms are available on the
Intervention Haiku.
If interventions are
not successful…
Contact parent and meet with your grade
level PLC. Complete Tier II Intervention.
Implement interventions for 4 - 6
weeks.
The SAT Chairperson will
schedule a Tier 3 meeting.
Implement interventions for 4 -6
weeks.
Meet with PLC to discuss results. If
interventions are unsuccessful, contact
parent and email paperwork to your SAT
Chairperson.
