The Heart Manual Service provides concise summaries of medical conditions to help patients better manage their health. It has manuals on conditions like heart disease, stroke, and cancer. The service has trained over 1,500 facilitators. Studies show the manuals improve health outcomes by enhancing consultations, reducing readmissions and visits, and improving medication adherence. New developments include digital formats and collaborations to create manuals for other conditions.
