Making referrals for home care
The growing demand for care As the aged in the population increases, as life expectancy increases, as those with chronic conditions increase, so will the need for care increase. The need for private duty home care continues to grow. Understanding how to incorporate private duty into your referral system will help both you and your patients.
Home Care Trend The trend towards in-home care is growing rapidly in the U.S. As people seek ways to avoid institutionalized care, the increase in home health care and non‑medical home services is quickly taking the place of traditional care options such as nursing homes.
The Continuum of Care The only choices for anyone who has a short or long term care need are either to stay in one’s own home with assistance or to move elsewhere: a family or friend’s home a nursing home an assisted living or continuing care situation
The best argument for staying at home with care is: Delivered correctly, home care is a humane and compassionate way to deliver care and supportive services. Home care can reinforce and supplement the care provided by family members and friends. Home care can help maintain the recipient's dignity and independence in the least restrictive care setting.
Private Duty Home Care The private duty home care model has grown to be a substantial part in the continuum of care, providing an alternative to institutionalization for those who understand who is eligible and how to make a correct referral for care.
“Because the Best Care is Home Care”® The benefits of long term in-home care Comfort Convenience Continuity Familiarity Safety Freedom Flexibility
Who utilizes private duty home care? Those with the financial resources to make choices for their care Those whose families accept the financial responsibilities of paying for care Those with long term care insurance coverage Those who use reverse mortgages or Veteran’s benefits to cover care
Home Care Models Home health agencies, non-medical home care agencies, and hospices are known collectively as “home care organizations”. Medicare's enactment in 1965 greatly accelerated the growth of in-home health care. The increased life span and the ensuing aging of America are creating a population with unprecedented needs for home care.
Referring Clients to Quality Private Duty Care The private duty industry is growing at a rapid rate and chances are that you will refer patients for private duty care. By understanding private duty services, you can help create a positive and safe experience for your patients who require home care not covered by public funds.
“Because the Best Care is Home Care”® Private duty care is provided in the comfortable and familiar surroundings of the patient’s home. Home care allows patients to retain independence and dignity. Home care allows patients to retain the continuity of established daily routines. Home is where family and friends can easily visit. Home is where most people prefer to live.
Activities of Daily Living (ADL) Examples Examples of non-medical services: Dressing Walking Eating Grooming Meal prep These fall under personal care and not non-medical: Bathing Transferring from bed or chair Toilet use
Instrumental Activities of Daily Living (IADL) Examples For non-medical services: Use of the telephone (look up numbers, dial, answer) Traveling via car or public transportation Food or clothes shopping (regardless of transport) Meal preparation Housework (laundry and light housekeeping)
Determining Care Needs The key determinant as to whether or not 24 hour care is needed is the patient’s ability to get up out of bed and to the toilet. If this cannot be done, then round-the-clock assistance is most certainly needed. Deciding on home care or institutional care at this point is a cost benefit analysis.
Personal Care Services Providing personal care services means providing a slightly higher level of care and helping patients with basic grooming and other “hands-on” Activities of Daily Living.
