The document describes Preventify.me, a tool to help caregivers stay on top of the health needs of their family members. It was created to address the problem that chronic diseases account for most deaths and healthcare spending in the US, but people have difficulty finding reliable and understandable health information. Preventify.me allows users to get tailored health recommendations, manage schedules and to-do lists for family members, send reminders and share information. It is designed based on health behavior theories and literacy principles to make information accessible across languages and literacy levels.
