• Visitation by lecturers
• Submit report and log

• Submit evaluation form
(filled by the employer)
• Assessment and
grading by coordinator

Finding LI Placement & Issue Application Letter
Submit Acceptance of Placement Form
Register in MAYA & Approval of Placement by Course
Coordinator
Report Duty to the LI Office
Submit Confirmation of Placement of LI
Undergo LI Period for Minimum 8 Weeks
Fill up log sheet for every week
Visitations by appointed lecturer to the LI office
Submit LI Report
Receive Evaluation Form from Employer
Final Assessment by Course Coordinator
