Subject: Pharmacy Liability Alert – Medication Therapy Management Dear Pharmacy Colleague, In a first-of-its-kind legal opinion, a federal court recently confirmed that pharmacists who conduct medication therapy management are held to a standard of practice equivalent to that of physicians who perform the same activities. And, when a pharmacist opts not to authorize a therapeutically indicated medication because the patient’s behaviors suggest the medication would be too risky, the pharmacist must document the reasons for not initiating the medication in the patient care record.
