Nutrigenomics: Can You Change Your Genes With What You Eat? is a quick review of the field, what it might do, and its benefits and risks, plus a short culinary medicine primer.
This document outlines an experiment to test if eating an apple a day keeps the doctor away. It involves randomly assigning 2000 participants (1000 males and 1000 females) to either eat one apple or a placebo apple per day for a year. The number of doctor visits each participant has will be recorded and compared between the apple-eating and placebo groups. Blocking by gender and double blinding both participants and evaluators aims to get a clear result on the effect of daily apple consumption while limiting bias. Concerns include difficulty finding and following all participants for a year and creating a believable placebo apple.
The document discusses the use of probiotics in preterm infants in the NICU to prevent necrotizing enterocolitis (NEC). It summarizes findings from several randomized controlled trials involving over 1400 preterm infants that compared administering probiotic supplements to placebo or no treatment. The studies found that probiotics were effective in reducing the risk of severe NEC and lowering mortality rates. Probiotics also decreased the number of days to full enteral feeds. While some safety concerns were noted, no trials reported probiotic-induced sepsis. Questions are raised about best probiotic preparations, dosages, and utilizing practices to promote colonization with maternal flora.
This randomized controlled trial tested the efficacy of the probiotic Lactobacillus reuteri in treating infantile colic in breastfed infants aged 2-16 weeks. It found that L. reuteri significantly reduced average daily crying time and increased the number of responders compared to placebo. Mechanisms may involve improving gut microbiota balance by reducing Escherichia coli and increasing Lactobacillus levels. The probiotic was well-tolerated with no significant differences in adverse events between groups. This study provides evidence that L. reuteri is an effective and safe treatment for infantile colic.
This study analyzed the Caesarean section (CS) rate in 300 women with type 1 diabetes (T1DM) who delivered at a large tertiary hospital using the Robson criteria for meaningful comparisons. The overall CS rate was 58% for women with T1DM, significantly higher than the institutional rate of 19.6%. Nulliparous women who underwent labor induction had a higher CS rate of 48.5% compared to 11.5% for multiparous women. Women with a previous CS usually required repeat CS for subsequent deliveries. High rates of labor induction, especially in nulliparous women, contributed to the high overall CS rates in women with T1DM.
This document discusses the rational use of medications during pregnancy and breastfeeding. It provides examples from clinical practice and aims to better recognize and recommend avoiding medications that pose risks to the fetus, whether they are prescription or over-the-counter. Several online evidence sources that can be used to classify medication risk during pregnancy and breastfeeding are also presented. A case study example involves advising a pregnant patient on treatment options for urinary symptoms.

On this call Drs. Ross and Desai presented evidence that prenatal exposure to EDCs, specifically the plastic component Bisphenol A, can increase the relative risk of adult obesity and metabolic syndrome. They also discussed the significance of these findings for neonatal and childhood care, and the need for strategies for the prevention and therapy of obesity to address programming effects of the early life environment.

Dr Ayman Ewies - The Effect of Exogenous Leptin Administration on Folliculoge...AymanEwies
Exogenous leptin administration in immature albino rats was found to enhance folliculogenesis, increase ovarian weight, and reduce total body weight compared to controls. Specifically, leptin-treated rats had a higher median number of antral follicles, significantly higher ovarian weights, and significantly lower total body weights than control rats across various ages. The results suggest leptin may play a role in female reproduction and metabolism, warranting further study of its potential effects in obese women with impaired reproductive functions.

Metagenics How Do I Quantify My Body and Try to Improve its Health? June 18 2019Larry Smarr
Larry Smarr discusses quantifying his body and health over time through extensive self-tracking. He measures various biomarkers through regular blood tests and analyzes his gut microbiome by sequencing stool samples. This revealed issues like chronic inflammation and an unhealthy microbiome. Smarr then took steps like a restricted eating window and increasing plant diversity in his diet, which reversed metabolic syndrome issues and correlated with shifts in his microbiome ecology. His goal is to continue precisely measuring factors like toxins, hormones, gut permeability and food/supplement impacts to further optimize his health.
Ahmed El-Sohemy is the chief scientific officer of Nutrigenomix Inc., a company that provides personalized nutrition recommendations based on genetic testing. The document discusses how genetic variations can impact an individual's response to different foods and nutrients. It provides several examples showing how coffee consumption and vitamin C intake may have different health effects depending on a person's genotype. Nutrigenomix offers direct-to-consumer genetic tests that analyze over 45 genes to provide personalized dietary and lifestyle advice. Studies show their DNA-based advice leads to greater understanding, interest in healthy eating, and long-term compliance compared to general population recommendations. Nutrigenomix is expanding globally and consumer demand for personalized nutrition is growing.

Daily consumption of blueberry bioactives for 6 weeks improved insulin sensitivity in obese, insulin-resistant adults compared to a placebo. A double-blind randomized clinical trial assigned 32 participants to consume either a smoothie with 22.5g blueberry bioactives or a placebo smoothie twice daily. Insulin sensitivity was measured using a clamp technique and increased more in the blueberry group compared to the placebo group without changes in body weight, energy intake, or inflammation markers. Blueberry bioactives may improve insulin action and reduce risk of diabetes.

Dr. Patrick Garrett is a chiropractor and clinical nutritionist who specializes in using lifestyle and dietary changes to reverse chronic health conditions. He believes that food can have a significant impact on mood, citing studies that have found many people with depression to be deficient in vitamins, minerals, omega-3 fatty acids and other nutrients found in whole foods like fruits, vegetables, nuts and seeds. Specific mood-boosting foods he recommends include cashews, citrus fruits, dark leafy greens, fatty fish and foods containing zinc, magnesium and B vitamins. His message is that making healthy dietary choices focused on nutrient-dense, whole foods can help improve mood and mental wellbeing.

Ultra-processing of food. Definition, consequences for health and emerging cr...Reijo Laatikainen
This document discusses ultra-processed foods and their health impacts according to the NOVA food classification system. It defines ultra-processed foods as industrial formulations containing ingredients like hydrogenated oils, high-fructose corn syrup, protein isolates, and artificial flavors. Studies show higher consumption of ultra-processed foods is associated with increased risks of obesity, heart disease, diabetes and early death. However, more clinical research is still needed to fully understand the health effects of ultra-processed foods. The NOVA system is useful for highlighting issues with modern food systems but may inaccurately classify some foods and confuse consumers.

He was diagnosed with an aggressive form of cancer in 1999 and told he likely only had 2 years to live with conventional treatment. Instead, Jerry researched cancer's causes and focused on strengthening his immune system by addressing nutritional deficiencies and high sugar consumption. He advocates following a diet rich in healthy fats and nutrients, like the Nourishing Traditions diet, to prevent and fight cancer naturally. Jerry founded a business providing organic remedies for livestock that don't use pharmaceuticals. He believes sustainable, grass-fed agriculture supports better health outcomes for farmers, animals, and consumers.

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).

3 Parts
What is Nutrigenomics?
What might it do for you?
How should you think about it?
4. Definition: ”an emerging field of research in
science which focuses on the effects
of foods and food constituents on gene
expression.”
Mission: “to optimize nutrition with respect to
an individual's genotype”
Vision: “accurate clinical tool available to every
family regardless of income”
(c) @johnlapuma CHEF Clinic.
I. What is Nutrigenomics?
5. • Nutrigenetics: how genes affect how we use
food and nutrition
• Epigenetics: how our environment, along with
nutrition and exercise, affects gene expression
• Nutrigenomics: how the nutrients we consume
affect our genes
(c) @johnlapuma CHEF Clinic.
Other terms
6. (c) @johnlapuma CHEF Clinic.
Genes are made of
DNA, and are
on chromosomes. 23
pairs of chromosomes
have ~20k protein-
coding genes.
7. Individuals with rare genetic disorders such as
phenylketonuria, galactosemia and hereditary
fructose intolerance as well as more common
disorders (e.g. lactose intolerance, celiac disease)
tailor food intake to bypass the metabolic/genetic
deficiency.

What are Specialty Fields in
Nutrigenomics?
1.Aging
2.Obesity
3.Cancer
4.Very specific amino acid, vitamin or
genetic defects, rare and unusual
In other words, advice should not be
one-size-fits-all.

What are Ways in Which
Nutrigenomics Is On Offer?
1.Microbiome Assessments (for the gut, of
course)
2.Genetic/genome assessments
3.Wearables and digital health…very little
data, but lots of ?s.
15. DNA Diet
ISOThrive - prebiotics for better poop
Nutrigenomix
PanTheryx- natural immunoglobulins
Second Biome- biotech/pharma
Second Genome
Ubiome- DNA test for poop
Viome- $1k/year for quarterly poop samples

DNA…now GeneDx, Ambry, Fulgent.
FDA Cease/desist 2015 changed offering to carrier
status and AR disorders (cystic fibrosis, Tay Sachs,
Sickle Cell)
In 2017, allowed variant analysis for late-onset
Alzheimer’s, Parkinson’s; 2018 for BRCA1 and
BRCA2…but not binary data, not dx, and 3/1000
genes.

24. And if you’re curious, more information, which
you can interpret yourself!
– promethease.com
•literature retrieval system that builds a
personal DNA report an unknown inborn
error of metabolism.
– snpedia.com
•Lots of errors and one interpretation.

Benefits: Tests give you
structure
27. Risk: potentially unnecessary surgeries/
avoidances/anxieties
Tell me again: eat fish instead of beef,
more protein versus less, nuts instead of
chips, drink tea instead of soda…isn’t that
what culinary medicine does?
(c) @johnlapuma CHEF Clinic.
Risks: Tests give you false positives,
and tell you what you already know.
28. Yes, if you have
– an unknown inborn error of metabolism.
– extra disposable income and are curious, and
not impetuous or anxious.
– an urge to read spreadsheets about your body.
No, if you’re concerned about data security,
privacy, wasted resources, or want to keep your
money and do what’s proven instead…

Testing Worth The Money?
29. 1. For obesity, for example.
2. Other common conditions…
e.g., diabetes
