MEMORANDUM OF UNDERSTANDING

This Memorandum of Understanding (MOU) is entered into on this day of 09/10/2023, by and between:
1. _________________________________________________, organized and existing under the laws of _____________________ Jurisdiction, with its
principal place of business located at ___________________________________________________________________________________, hereinafter referred to as the "Hospital."
2. _________________________________________________, a [Legal Status] organized and existing under the laws of ________________, with its
principal place of business located at [Lab Address], hereinafter referred to as the "Diagnosis Lab."
The Hospital and the Diagnosis Lab may be collectively referred to as the "Parties."
WHEREAS, the Hospital provides reliable diagnostic testing services to the patients and requires support for sample collection from diagnosis lab.
WHEREAS, the Diagnosis Lab specializes in acquiring customer and providing required samples to the hospital for processing diagnostic testing services and
has the necessary infrastructure, equipment, and qualified personnel to perform various sample collection.

2. Purpose of Collaboration:
• NOW, THEREFORE, the Parties hereby agree to the following terms and
conditions:

• The Hospital and the Diagnosis Lab intend to collaborate for the provision
of high-quality diagnostic testing services to the patients of the diagnosis
lab.
• 2. Scope of Services:

to provide the following diagnostic testing services to the Hospital:

3. Responsibilities

• 3.1 The Diagnosis lab shall:
• - collect required samples from patients to the hospital for diagnostic testing services as needed.
• - Provide necessary patient information and samples for testing.
• - Ensure compliance with all legal and regulatory requirements.

• 3.2 The Hospital shall:
• - Perform diagnostic tests accurately and in a timely manner.
• - Maintain confidentiality of patient information.
• - Adhere to industry standards and relevant regulations.
• - Share test results promptly with the Hospital for patient care.
• 4. Quality Assurance:
• Both Parties agree to maintain the highest standards of quality in their respective services and collaborate to continuously
improve the quality of diagnostic testing and patient care.

4. Fees and Payments
• Fees and Payments:
• The Parties shall agree on the fee structure for the diagnostic testing
services provided by the Hospital. The terms of payment, including
billing and reimbursement, shall be defined in a separate agreement.
• 6. Duration:
• This MOU shall commence on the effective date and remain in force
for a period of 6 months and will get terminated either earlier by
mutual agreement or for any other valid reason or automatically if
not renewed.

5. Termination:

• Either Party may terminate this MOU upon 15days written notice to the other Party. Termination shall not affect the completion of services for patients already referred to the
Hospital.

• This MOU shall be governed by and construed in accordance with the laws of Rajmundry Jurisdiction.
