The Hospital and the Diagnosis Lab may be collectively referred to as the "Parties."
WHEREAS, the Hospital provides reliable diagnostic testing services to the patients and requires support for sample collection from diagnosis lab.
WHEREAS, the Diagnosis Lab specializes in acquiring customer and providing required samples to the hospital for processing diagnostic testing services and
has the necessary infrastructure, equipment, and qualified personnel to perform various sample collection.
2. Purpose of Collaboration:

conditions:

• The Hospital and the Diagnosis Lab intend to collaborate for the provision
of high-quality diagnostic testing services to the patients of the diagnosis

services provided by the Hospital. The terms of payment, including
billing and reimbursement, shall be defined in a separate agreement.

for a period of 6 months and will get terminated either earlier by
mutual agreement or for any other valid reason or automatically if
not renewed.
