The route to success in end of life care - achieving quality in care homes

This guide follows the six steps of the pathway laid out in the national End of Life Care Strategy.
It includes questions staff and managers should ask about end of life care provided in their care home and the employees' role in that care.
The guide is linked to the End of Life Care Strategy Quality Markers.

As part of the Initial Discharge Planning Assessment, each PCC was asked to give a blessing at the bedside with the patient or to add a moment of brief kind spoken words during completion of all assessments. The goal of this project was to create a transpersonal caring moment between patient and nurse with focus upon use of Caritas Process #4, Creation of a trusting, caring relationship between the nurse and patient in establishing a comprehensive discharge plan.

The report that captures the outcomes and spirit of the #ProjectA Accelerated Design Event (February 14th 2019). Improving the ambulance service response to mental ill health and emotional distress.
iCamp: How might we help people to understand about Mental Health?
This iCamp was a particularly emotional one. During an iCamp, participants share their experiences. The men and women in the room shared real stories of mental health struggles that impacted family members, friends, and themselves.
The room was part brainstorm, part group therapy, as people spoke about the difficulties of finding the right care, of being targeted by judgemental people in their communities, and of the emotional toll of dealing with a loved one that was struggling, for example.
One story that was particularly striking involved a participant telling the story of how it took her family twenty years to find the right care for her family member.
Summary from the very first Capital C event held at Impact Hub Kings Cross on Saturday 29th November.
Capital C is a collaboration to improve cancer care for the people of London hosted by Macmillan Cancer Support and Swarm. The goal for the group is to put patient's voice at the heart of a long-term strategy to improve patient experience in London.

How does design thinking change mindsets and culture so that a nation like Singapore can continue to innovate and lead the transformation for a better future? Harnessing the power of design thinking to build a ‘Smart Nation’ for Singapore, this workshop aims to give audiences the opportunity to realize the potential this methodology has.
A user-centered way of solving problems, design thinking involves collaboration across user segments, through strategies like customer journey mapping, design research and rapid prototyping. While design is often used to describe an end-product, in reality if applied properly, can be used to address problems or issues across a variety of field including social issues.

I was pleased to be involved in some of the interviews that happened when the authors were planning this report... and then to be asked to write the Preface was an honour. Since publication I am not myself very aware of the impact of it, and I think this is such an important piece of work, that it deserves publicity as widely as possible. Hence uploaded here
The MandMade Family programme successfully supported men to be able to talk more openly about their emotions, to build their confidence and self esteem, to know where to go for help and to support others in the community.
This was achieved through an eight week workshop programme which includes peer discussion, information sharing and self-reflection on a range of health and wellbeing topics, underpinned by person centred facilitation approaches.
This report presents findings of an evaluation of The ManMade Family programme, delivered in Sandwell from February – March 2016. ManMade is an eight week programme of workshop sessions designed to support and empower unemployed men to take care of their own mental health and wellbeing. It was developed by Forward for Life and Common Unity in response to high levels of poor male mental health and suicide, associated with gender identity.

Practical Playbook Steering Committee Members Lloyd Michener, MD and Denise Koo, MD, MPH presented "A Perfect Storm for Population Health" at APTR's 2015 Teaching Prevention conference. The presentation helped described the forces that are coming together for population health improvement and the opportunities that are enabling these partnerships to succeed; and discussed innovative tools for those in the field to utilize in their population health efforts.
