Fluid Lavage of Open Wounds (FLOW)
enrols 2,545 pa ents at 41 sites in
Canada, United States, Australia,
Europe, and India, making FLOW the
largest orthopaedic trauma study
conducted to date.
PARITY trial receives defini ve
funding from CIHR making it the first
mul -na onal sarcoma study to date.

Orthopaedic Residency Educa on
Training Program
A dedication to training surgeons
The MacOrtho residency program con nues to
collaborate and forge ahead in the areas of educa on
and research. We have developed collabora ons with
the Research and High Performance Compu ng team
with Ranil Sonnadara and are formula ng specific
procedural based competencies and evalua on tools.
This is in keeping with the Royal College mandate of
“Milestones 2015” and the further development of a
more formal competency based program.
We have remained ac ve again this year at the Royal
College’s Interna onal Conference on Residency
Educa on (ICRE). Dr. Brad Petrisor was involved in
the planning and delivery of the annual “New
Program Directors workshop” which received
posi ve reviews and MacOrtho educa onal work was
presented at the main mee ng of the ICRE.
We have a stellar group of residents and will be
looking to add to our cohort with the upcoming
CaRMS cycle. We con nue to prepare for our
upcoming formal Royal College review which is
scheduled to take place in April of 2015.
Our program con nues to excel with the goals and
vision of both the Clerkship and Mentorship
programs. Our Fellowship program con nues to
draw excellent cohorts both na onally and
interna onally, further branding MacOrtho as a
leader in evidence-based orthopaedic surgery.

16. Clerkship Program
Here in the Division of Orthopaedic Surgery at McMaster University, we recognize the importance of engaging
students at every level of educa on. Moving forward, our main goal is to iden fy various ways to further
inspire and prepare clerks to pursue a career in orthopaedic surgery.
The success of our Mentorship Program will lead to a strengthening of rela onships between
orthopaedic surgeons and trainees, aid in conflict resolu on, assist in career planning resul ng in overall

their leadership journey. The mandate of this series is to
provide key insights on true leadership, trials and
tribula ons faced on the road to success, strategies for

20. ABC Travelling Fellowship

The historical visit of the American-Bri sh-Canadian
Travelling Fellows to McMaster University took
place June 15-18th, 2014. Apart from their arrival
and departure days, the fellows spent 2 full days
with MacOrtho faculty and residents.
The fellows, Pramod Achan (England), Will Aston
(England), Rob Ashford (England), Brendan Coleman
(New Zealand), Greg Firth (South Africa), Sanjeev
Pa l (Scotland) and Phong Tran (Australia), began
their tour with an outstanding academic session on
the morning of Monday, June 16th in the Great Hall
of the University Club. In front of a packed room,
the fellows presented their research that closely
aligned with those of MacOrtho. In turn, the
MacOrtho faculty presented research studies
represen ng similar interests, thus genera ng
enthusias c and collabora ve discussion.
Lunch in the University Club was accompanied by
further academic discussion and tableside chats
with our Academic Head, Mohit Bhandari. The
a ernoon was spent with subspecialty groups and
further explora on of common surgical interests
and research collabora ons. The fellows then
spent the evening with MacOrtho families at a
poolside BBQ, enjoying the relaxed environment
and camaraderie. The fellows enjoyed wine tas ng
tours at Hillebrand and Peller Estates, and re-
invigorated with a dousing of cool water on the
Hornblower through Niagara Falls.
Official feedback from the COA/AOA regarding the
ABC Travelling Fellowship visit in June of this year
reported scores in 2 categories: educa onal
experience and accommoda on/social hos ng.
Ourcombined scores ed for first place among the
11 elite ins tu ons visited by the Fellows, including
Harvard, Duke, Vanderbilt, Rothman Ins tute,
Pi sburgh and the University of Toronto.
Most notably, McMaster was the only ins tu on to
receive a perfect score in the ‘educa onal
experience’ category.

26. Current Research in
Orthopaedic Surgery
Our current research ini a ves revolve around 5 major areas: Trauma &
Injury, Degenera ve Joint Disease, Musculoskeletal Tumour Surgery, Sports
Medicine and Disorders of Spine. The ra onale for the five major themes,
each with their unique foci, is driven by the increasing global burden of
orthopaedic trauma injuries worldwide due to the prevalence of road traffic
injuries, the crisis of inten onal injuries in women from domes c violence,
the epidemic of fragility fractures in the elderly, the high rates of
osteoarthri s, the devasta ng effects of bone cancer, the debilita ng
consequences of an orthopaedic sports injury, and high level of disabili es
resul ng from spinal injuries and condi ons. These debilita ng injuries and
disorders affect people of all ages, from children to the elderly. Each of these
core research themes are opera onalized primarily by the conduct of mul -
na onal defini ve randomized controlled trials and large observa onal cohort
studies.

Road Traffic Accidents &
Associated High-Energy Trauma
Trauma is the leading cause of death in the first
four decades of life, surpassed only by cancer and
atherosclerosis, as the major cause of death in all
age groups. For every person killed as a result of
an injury, threefold more survive and are le
permanently disabled. Musculoskeletal injuries,
such as fractures and disloca ons, are very

randomized design to compare two surgical

(vitamin D3 4,000 interna onal units per day for
six months versus placebo) on pa ent important
complica ons and quality of life in 807 non-
geriatric (ages 18 to 60 years) femoral neck
fracture pa ents.

Our proposed cross-sec onal study aims to assess
the burden of distracted driving on pa ents who
present to orthopaedic fracture clinics.
Specifically, the primary aims of the study are to
determine: (1) what propor on of pa ents
present with a musculoskeletal injury that was the
direct or indirect result of distracted driving; and
(2) what types and severity of injuries can
the direct or indirect result of distracted driving;
and (2) what types and severity of injuries can
distracted driving lead to. Our secondary aims
include evalua ng (1) the types and frequency
of distracted driving in fracture pa ents and (2)
the types and frequency of distracted driving
witnessed by fracture pa ents in other drivers.

Inten onal Injuries in Women
The World Health Organiza on reports that one
in three women globally will experience physical
or sexual in mate partner violence (IPV) or
domes c abuse in their lives. Every six days, a
woman in Canada is killed by her in mate partner.
In mate partner violence is a prevalent social
issue which poses significant health concerns. It
dispropor onately affects women and is a leading
cause of non-fatal injury in females in North
America. The cost of IPV in Canada is es mated at
$5 billion annually. Vic ms of abuse experience
more physical and mental health problems,
including orthopaedic injuries and, consequently,
use health care resources more frequently than
non-abused women.

control phase consists of surveying pa ents who
visit the orthopaedic clinic about their percep on
of this environment as an open and resource-rich
place in which to discuss IPV, should the need
arise. The interven onal phase will consist of
the implementa on of our self-designed IPV
media 'toolkit', which includes several versions
of a poster, an informa onal brochure with card
insert, and staff bu ons to be worn during the
normal opera ng hours of the orthopaedic
clinic.

Fragility Fractures in Elderly
Fragility fractures are common problems in
Canada and have a significant economic impact
on the healthcare system. It is es mated that at
least one in three women and one in five men will
suffer a fragility fracture in their life me and the
cost of trea ng osteoporo c fractures in Canada
is more than $1.3 billion dollars per year. Two
common types of fragility fractures are hip
fractures and shoulder fractures.

39. FAITH is a large mul -centre randomized controlled
trial comparing sliding hip screw and cancellous
screw fixa on in femoral neck fracture pa ents.
Surgeons will use 1 of 2 surgical strategies in 1,500
pa ents who have sustained a femoral neck
fracture. The outcomes measured are revision
surgery rates and quality of life up to 24 months
post ini al hip surgery.
The secondary objec ve is to evaluate the impact of
sliding hip screws versus cancellous screws on
complica ons, including mortality, avascular necrosis,
nonunion, implant breakage or failure, and infec on.
Over 70 sites are currently par cipa ng in the FAITH
study: Canada (12 sites), USA (41 sites), Netherlands
(14 sites), Norway (1 site), Australia (3 sites), and
India (11 sites).

Malignant bone tumours (bone sarcomas) o en
affect adolescents and young adults. In Canada,
bone sarcomas affect 4 to 5 pa ents per million
persons each year. Long-bone sarcomas were
historically managed with amputa ons, but the
current standard is limb salvage surgery, which
involves wide tumour resec on followed by
func onal reconstruc on. With the aid of modern
imaging techniques and chemotherapeu c
regimens, limb salvage is possible in 95%
of pa ents.

randomized controlled trial which will evaluate the
efficacy of short versus long-term post-opera ve
prophylac c an bio c regimens on the incidence of
deep infec on following endoprosthe c
reconstruc on of lower extremity bone tumors. This
is, to our knowledge, the first randomized controlled
trial in orthopaedic surgical oncology and will include
920 pa ents recruited by surgeons in Canada, the
United States and Argen na. The PARITY trial is
demonstra ng early success by enrolling 34 pa ents,
which is one-third of the feasibility sample of 100
pa ents earlier than expected.

46. Sports Medicine
Sports-related injuries account for 30 to 40
percent of all uninten onal injuries. Sports
accidents are the leading cause of injury in young
adults. With approximately 30 million children
and adolescents par cipa ng in sports ac vi es
according to the U.S. Centers for Disease Control,
the prevalence of sports injury is on the rise.
Nearly 40 percent of sports injuries occur in
children aged 5 to 14 years, and the severity of
the injuries increase with age. High school aged
individuals account for 2 million injuries that
result in over 500,000 doctor visits and 30,000
hospitaliza ons in the United States. The
prevalence is higher amongst the younger age
groups as a result of immature bones, insufficient
training, and overuse injuries.

We have a 30-day TJR readmission rate which is half that of the best reported
rates in the US. We a ribute this to our prac ce protocols which we have been
instrumental in establishing to improve the pa ent experience.
The Hamilton Arthroplasty Group (HAGrp) at the Hamilton Health Sciences Juravinski Hospital is an
independently funded mul disciplinary clinical outcomes research group dedicated to improving total joint
replacement outcomes, specifically pa ent quality of life through con nuous quality improvement in clinical
service, research and educa on in total joint replacement (TJR). In 2013/14 our focus was on research
ini a ves addressing preopera ve pa ent factors, including medical condi ons/comorbidi es and
psychosocial factors that affect TJR pa ent outcome, specifically looking at how these impact such quality
based procedure outcomes as hospital length of stay.
In 2014/15 the Hamilton Arthroplasty Group con nues to focus on pa ent outcomes as well as collabora ve
issues with other research groups across various disciplines. Of note is a key quality assurance trial set to begin
in 2015 in collabora on with Infec ous Diseases to determine the effec veness of a preopera ve
Staphylococcus aureus screening and decoloniza on program to reduce the rela ve risk of prosthe c joint

MacOrtho hosted the second annual
Femoroacetabular Impingement symposium on May 3,
2013. This second mee ng was mostly interac ve,
fostering discussion regarding current prac ces and
challenges with a focus on future research ini a ves
within our FAI collabora ve.
During the morning session, selected speakers
presented their current FAI research. In the a ernoon
session, an Inves gators Mee ng was held for the
Femoroacetabular Impingement Randomised
Controlled Trial (FIRST), led by Dr. Olufemi Ayeni. The
FIRST trial will inves gate whether surgical correc on
of the impingement (arthroscopic osteochondroplasty)
provides improved clinical results (decreased pain and
increased func on) at 12 months compared to
arthroscopic lavage of the hip joint in pa ents with
FAI. Enrolment of 50 pa ents in the FIRST pilot study
