16. Breast Self Examination (BSE)
• Opportunity for woman to
become familiar with her
breasts
• Monthly exam of the
breasts and underarm area
• May discover any changes
early
• Begin at age 20, continue
monthly

• More frequent
examination for high risk

● Imaging of choice for <40
years old
● Its combination with
mammography can have a
specificity of up to 95%
● Asian breasts usually have
dense breasts
● Quick, non-invasive, painless
and inexpensive imaging test
30. Surveillance
● Checkup every 3-6 months
● Annual Mammogram and Breast Ultrasound
● As needed:

Breast cancer is the most common cancer in women worldwide
Did you know that the Philippines has the highest incidence rate of breast cancer in Asia? and the 9th highest in the world
it is the most common type of cancer followed by lung and colon. and there are almost 28thousand newly diagnosed cases of breast cancer in 2020, and it is the 3rd most common cause of cancer death.
Fact # 1 All women are at risk. Approximately 70% of breast cancers occur in women with none of the known risk factors.Fact # 2 Only about 5% of breast cancers are inherited. About 80% of women diagnosed with breast cancer will be the first to be victims in their families.Fact # 3 Early detection does NOT prevent breast cancer. Avoiding and elimating known causes will prevent breast cancer!Fact # 4 Mammography fails to detect as much as 20% of all breast cancer and as much as 40% in women under the age of 50.Fact # 5 One out of four who are diagnosed with breast cancer die within the first five years. No less than 40% die within ten years.Fact # 6 Risk factors are not necessarily causes of breast cancer. Enough evidence exist linking environmental pollution and contamination to cause breast cancer.Fact # 7 The global incidence of breast cancer has tremendously risen by 256% the past 30 years with Southeast Asia increasing by 479%! And the supposed authorities and experts that should know, don't know why.Fact # 8 Breast cancer is the leading killer of women ages 35 to 54 worldwide. More than a million women develop breast cancer without knowing it and almost 500,000 die from it every year. Fact # 9 1 out of 13 Filipno women will develop breast cancer in her lifetime. The province of Pampanga has the highest incidence rate in the entire country.Fact # 10 The Philippines has the highest incidence rate of breast cancer in Asia and registered the highest increase of 589% among 187 countries over a 30 year period from 1980 to 2010.
Not all tumor is cancer– it can be benign or malignant.
Cancer, it has the potential to spread or metastasize
(In the Philippines, surgeon--specifically breast surgeon who manages breast diseases)
0, there is cancer but no palpable mass, usually detected in screening mammography
Stage 1 and 2, maliiit na bukol
Stage III kalat sa axillay nodes
Stage IV distand spread, bones, lung, liver
Why do people develop breast cancer
Risk factor is anything that increases risk to develop a certain disease
Unfortunately, most important risk factors are NONMODIFIABLE:
Female
Age
Family and genetics
Medical history
Good thing, there are modifiable risk factors.
Aside from palpating a lump on your breast, these are other symptoms
2 components
Low detection rate
However, A breast self-exam isn't a substitute for a breast exam by your doctor (clinical breast exam) or a screening mammogram. Sensitivity or detection rate 26% only
A breast self-exam isn't a substitute for a breast exam by your doctor (clinical breast exam) or a screening mammogram.
Mammography has been shown in studies to decrease your chance of dying from breast cancer by 20-40%.
It’s a type of xray, with minimal dose of radiation, not cancer causing
Why 40?1. breast ca more common women age 40 above, 2. young patients usually have dense breast.
And if you have a relative who had breast cancer, you should start screening mammography 10 years before the age of diagnosis.
When you do a self breast exam your chances of detecting a mass is 26% but it increases/doubles when an expert does the breast examination, however, it increases up to almost 90% when you combine it with imaging in the form of mammography
The sensitivity and specificity of mammography decreases with the density of the breast
What do we do when there is a lesion?
New method: vacuum assisted excision biopsy– however, it is available only in certain large hospitals in the country
Adjuvant treatments are equally important..
Depends on the stage and subtype of breast cancer
Usually if advanced stage, a combination of all this, multidisciplinary
Surgery for breast is usually 2 steps, in 1 OR setting po, for the breast and for the axilla
If the patient does not want a flat chest,
surveillance is very important, because it can always recur, even if you havce the best doctors, they can’t guarantee 100%.
This table is just to show you that there is really benefit in detecting breast cancer early. Our goal is to detect breast cancer as early as possible.
Patients diagnosed with breast cancer, there is hope! It’s not a death sentence.
Short and frank answer, is actually, yes and No, BUT 1. we can do ways to decrease our risk 2. we can detect it early
