This document describes a clinical case involving a 40-year-old pregnant woman in her 28th week of pregnancy who is complaining of a slow-healing wound on her leg. Her medical history includes a previous delivery at home with a midwife where the baby died after birth. On examination, she has edema in her lower extremities and an ulcer measuring 2.5 cm on her leg that is secreting pus.
2. KASUS
Seorang perempuan umur 40 tahun hamil anak kedua usia kehamilan
28 minggu mengeluh ada luka di kaki yang lama sembuh. Riwayat
persalinan sebelumnya pernah melahirkan bayi besar ditolong oleh
dukun di rumah dengan kondisi bayi meninggal setelah lahir. Riwayat
penyakit keluarga : DM (ibu kandung), Hasil pemeriksaan LILA : 26
cm, tinggi badan 155 cm, BB 62 kg (BB sebelum hamil 55 Kg). Tanda
tanda vital : KU baik, TD : 140/90 mmHg, Nadi: 82 x/menit,
pernapasan : 20 x/menit, Suhu : 37,2 ˚C.
Hasil pemeriksaan fisik: konjungtiva merah muda, sklera putih, muka
tidak oedema, tidak ada pembesaran kelenjar tiroid, areola mamae
kotor, ekstremitas atas : tangan kebas, oedema negatif.
Hasil pemeriksaan abdomen : inspeksi abdomen : pembesaran lebih
besar dari usia kehamilan, Leopold I : TFU 28 cm (3 jari di atas
pusat),Leopold II : Punggung kiri, Leopold III : Presentasi Kepala,
Leopold IV : Belum masuk PAP, Auskultasi DJJ : 130 x/menit teratur.
Ekstremitas bawah : tungkai kebas, oedema (+), reflek patela (+)/(+),
terdapat ulkus dengan diameter + 2,5 cm, terdapat PUS pada luka.
