The Pioneer Valley Information Exchange (PVIX) was formed by Baystate Health and other regional healthcare providers to build a new health information exchange (HIE) in Western Massachusetts to help coordinate care and support accountable care models. PVIX selected InterSystems HealthShare as its technology platform and partnered with J2 Interactive for implementation. J2 helped launch the PVIX exchange within seven months and integrated Baystate Health as the first stakeholder. PVIX now provides a platform for physician alignment, data analytics to support integrated care strategies, and value-added services to mitigate risk across the region.

In January 2013, Catholic Health Initiatives began a multi-phase journey to develop a population health management solution across all of its regions. This presentation will describe the strategies the health system pursued for: creating a clinically integrated network as a first step in managing the health of populations and integrating care across the patient experience; aligning hospitals and physician groups to create successful clinical models; creating a data platform to share clinical measures and benchmarks; and ultimately becoming a risk-bearing shared savings ACO. Participants will hear real-world examples of best practices for how to meet FTC regulations, create an effective governance structure to manage performance, and align financial incentives. Learn how one of the nation's largest hospital systems developed a system-wide population health management solution in order to achieve the necessary transformation from fee-for-service to fee-for-value.

More and more healthcare organizations are recognizing that clinical integration of providers is a prerequisite to care coordination, population health management, and accountable care organizations. Baystate Health and Its Regional Partners Call Upon J2
to Help Build a New HIE For Western Massachusetts
Faced with the challenge of implementing accountable care and risk-sharing
reimbursement models across Western Massachusetts’ growing provider community,
a consortium of healthcare providers and organizations led by Baystate Health and
Baycare Health Partners formed the Pioneer Valley Information Exchange (PVIX).
Having chosen InterSystems HealthShare as their technology platform, and
J2 Interactive as their strategic implementation partner, the PVIX team integrated
Baystate Health as the first stakeholder for the new exchange. J2, Baystate Health,
and InterSystems worked in close collaboration to complete a successful launch of
the PVIX exchange within seven months of project initiation.
In addition to meeting Meaningful Use Stage 1 and 2 requirements, J2’s work with
HealthShare is enabling PVIX to provide:
ƒƒ A comprehensive, EMR-agnostic platform for information exchange and interoperability,
achieving the goal of physician alignment across the region
ƒƒ The data aggregation and analytics foundation necessary to support integrated healthcare
