Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

w Introduzione
L
a patologia occlusale come viene defini-
ta da Dawson è un processo distruttivo
progressivo a carico di denti e articola-
zioni temporo mandibolari, risultato di una
incoordinazione tra queste due strutture per
mezzo delle forze esercitate dai muscoli
masticatori [1]. Tale affezione risulta essere
molto frequente, secondo il succitato auto-
re la più diffusa delle patologie dell’appa-
rato stomatognatico. Uno degli effetti più
facilmente evidenziabile è l’usura precoce
degli elementi dentari. Questo fenomeno
oltre che indebolire la struttura dentaria, si
ripercuote anche sull’estetica del sorriso e
sulle dinamiche funzionali e parafunzionali.
Ripristinare una corretta occlusione dunque
ha una valenza non solamente estetica,
generalmente primum movens portato dal
paziente, ma anche e soprattutto un razio-
nale di prevenzione nei confronti di una
degenerazione progressivamente più grave
di ATM e denti. La possibilità oggigiorno
di avvalersi di tecniche affatto invasive che
eliminino completamente la necessità di pre-
parare quegli elementi che già hanno subito
perdita di sostanza preservando in maniera
massimale il substrato migliore per l’adesio-
ne dei restauri, quale lo smalto, rappresenta
la scelta terapeutica più lungimirante.
Questo articolo, attraverso la descrizione
di un caso clinico, illustra i passaggi fonda-
mentali di una tecnica riabilitativa comple-
tamente addizionale realizzata in ceramica
feldspatica in cui l’aumento della dimensio-
ne verticale consente di ottenere lo spazio
necessario per la realizzazione di veneer-
lay e allo stesso tempo di ridurre la ripidità
della guida anteriore.
w Un approccio ultraconservativo
L’usura precoce dei denti è una disfunzione
del sistema masticatorio che può presentarsi
sotto diverse forme in funzione delle diffe-
renti cause che la determinano [2,3,4]. Di
qui ne deriva che un tale fenomeno neces-
sita di un inquadramento corretto che parta
da un’attenta osservazione delle faccette di
usura per individuare la causa e quindi im-
postare il piano terapeutico. Nel caso qui
presentato l’usura della superficie linguale
degli incisivi superiori e dei margini incisali
di quelli inferiori lascia supporre un pattern
funzionale anteriorizzato per la presenza di
interferenze bilancianti nelle lateralitá rap-
presentate dai terzi molari inferiori fuori dal
piano occlusale (Figg. da 1 a 4).
Inoltre la presenza di una Curva di Spee
accentuata e il conseguente aumento dell’o-
verbite rendevano la situazione occlusale
del paziente decisamente a rischio di svi-
luppo di disfunzioni temporo mandibolari.
Le abfractions rilevate a livello dei colletti
di 2.3, 1.4 e 4.4 e l’usura del margine inci-
sale degli incisivi superiori testimoniavano
la presenza di parafunzioni con stress note-
voli a carico degli elementi. La presenza di
“conche” sulle superfici occlusali e i margini
incisali di molti elementi lasciavano inoltre
sospettare la sovrapposizione di fenomeni
erosivi ad accelerare il processo distruttivo
della dentatura. Il paziente si presentava
con doppio click reciproco in apertura e
chiusura sia a destra che a sinistra, dolenzia
alla palpazione dell’ATM di destra, cefalea
tensiva in regione temporale.
2. Protesica

Il piano di trattamento dunque necessaria-
mente andava organizzato in modo da ve-
rificare la nuova dimensione verticale solo
dopo aver risolto la dolenzia dell’ATM. La
letteratura scientifica in merito ha subito un
drastico cambiamento di tendenza negli
ultimi anni. Se infatti la DVO (dimensione
verticale dell’occlusione) risultava un tempo
un parametro immodificabile da ricercare
con esattezza, testare e riprodurre fedel-
mente nelle riabilitazioni definitive, recenti
articoli mostrano che una variazione di 5
mm può essere ben tollerata dal sistema
senza recare danni nè alle articolazioni nè
ai muscoli [5,6,7,8,9,10,11]. Poter modificare
la DVO aumentandola in quei pazienti con
morso profondo e usura del gruppo fronta-
le consente di poter ridurre l’overbite e con-
testualmente ripristinare la morfologia degli
elementi anteriori. Ora questo può ottenersi
in 2 modi. Il primo, attraverso un esclusivo
allungamento degli incisivi inferiori; questa
opzione, peró, presenta diversi punti sfavo-
revoli: non riduce l’overbite, non riduce la
ripidità della guida incisiva, porta il mar-
gine degli incisivi inferiori oltre lo stomion
[12]. Il secondo, con un prevalente interven-
to a carico del cingolo degli incisivi superio-
ri. Questa seconda soluzione offre di contro
i vantaggi di ridurre l’inclinazione della gui-
da incisiva, ridurre l’overbite, mantenere il
margine degli incisivi in un’area funzionale
corretta. In questo caso si è dunque deci-
so di lavorare su entrambi i fronti volendo
comunque ripristinare la morfologia degli
elementi inferiori decisamente coinvolti dal
processo abrasivo (Figg. 5 e 6). Avendo
il paziente un processo infiammatorio in
corso a carico dell’ATM si é optato per un
percorso terapeutico che valutasse la nuova
dimensione attraverso un presidio reversibi-
le quale una placca di Michigan superiore
portata 24 ore su 24 per 3 mesi (Fig. 7). Al
termine di questo periodo è stata eseguita

stato di abrasione moderata diffusa

una nuova valutazione delle articolazioni le
quali, pur mantenendo un click reciproco,
non risultavano dolenti alla palpazione e
alla prova di carico. Un tale quadro inco-
raggiava il completamento dell’iter terapeu-
tico a questo punto con modifiche a carico
degli elementi come di seguito descritto.
w Sequenza terapeutica
Dopo un periodo di stabilizzazione arti-
colare attraverso bite di svincolo superiore
per 3 mesi ed estrazione degli ottavi che
per la loro posizione creavano interferenze
bilancianti importanti, si è proceduto a re-
gistrare la nuova relazione centrica. Come
riferimento per la cera di montaggio è stato
preso il rialzo creato dal bite misurato con
calibro a livello dei colletti di 13 e 33 (Fig.
8). Altre 2 cere di centrica ad una posizione
di +1 mm e -1 mm sono state rilevate (Fig.
9). Una tale ridondanza di registrazioni
consente di testare la validitá del montag-
gio. Dopo aver infatti utilizzato la cera di
riferimento per il montaggio in articolatore,
si utilizzano le restanti 2 per verificare l’asse
cerniera, controllando che si adattino per-
fettamente ai modelli alle diverse altezze.
Questo passaggio consente di eseguire con
una certa tranquillità variazioni di rialzo
della DVO, riducendo al minimo il rischio di
errori grossolani.
Una volta montato il caso e valutate le fo-
tografie pre intervento, si è programmato
un aumento della DVO che consentisse la
realizzazione di veneerlay in ceramica di
spessore adeguato nei settori posteriori in-
feriori corrispondente ad un rialzo registra-
to sull’asta incisale di 3,5mm. Nel settore

frontale il gap doveva essere colmato a
carico del versante palatale dei superiori
attraverso ricostruzioni dirette in composi-
to guidate dalla ceratura, a livello inferiore
con delle additional veneers in ceramica
feldspatica. Il rialzo veniva realizzato, dun-
que, intervenendo esclusivamente sugli ele-
menti inferiori laddove il ripristino estetico
e delle funzioni di guida canina e incisiva
si sarebbe completato con additional vene-
ers in ceramica realizzate da 1.4 a 2.4. Il
momento più critico di tutta la procedura
sarebbe stato la provvisorizzazione che in
una riabilitazione senza preparazioni degli
elementi presenta problematiche quali frat-
tura e decementazione.
Volendo comunque testare in modo fisso
la nuova occlusione riducendo al minimo i
disagi legati alla perdita continua di pezzi
si è deciso di optare per una provvisoriz-
zazione in bisacrilato stampata direttamen-
te sul paziente previo pretrattamento dello
smalto con mordenzatura a spot e adesivo.
Un tale materiale, grazie alla sua elasticità
e alla sua forte adesione agli adesivi per
compositi, risulta essere un ottimo materiale
in queste situazioni. Il rovescio della meda-
glia è rappresentato dalla difficile rimozio-
ne soprattutto del bond sui punti di smalto
mordenzati.
Per ridurre al minimo i passaggi, si è deciso
di agire con la seguente sequenza: realiz-
zazione di mascherina in silicone traspa-
rente con supporto rigido realizzata sulla
ceratura dei cingoli degli elementi frontali
superiori e 4 cucchiai individuali 2 superiori
e 2 inferiori, mascherina in silicone non tra-
sparente ad elevata rigidità di tutta la cera-
tura nell’arcata inferiore (Figg. da 10 a 12).
Nella seduta su paziente si procedeva al
trattamento delle superfici palatali degli
elementi da 1.3 a 2.3 e all’applicazione
di composito, quindi posizionamento della
mascherina in silicone trasparente, rimozio-
ne degli eccessi e fotopolimerizzazione. A
completare la procedura, si rifiniva con fre-
se multilama e si lucidava con gommini. Si
andava quindi a rimuovere tutte le zone di
sottosquadro e ad ammorbidire gli spigoli
vivi per completare la procedura di prepa-
razione degli elementi eseguendo il bon-
ding di tutta la dentina esposta e il build up
dei colletti (Figg. da 13 a 15). A questo pun-

to si prendevano le impronte di precisione.
La scelta di procedere in questa fase con le
impronte definitive nasceva dalla necessità
di ridurre al minimo le sedute di rimozione
dei provvisori che, come detto in preceden-
za, risulta essere relativamente indaginoso.
Veniva presa una cera per il cross mounting
e quindi si realizzava il provvisorio inferiore
trattando con mordenzatura a spot e bond
gli elementi coinvolti per poi stampare il bi-
sacrilato e polimerizzare (Figg. 16 e 17).
Sui modelli definitivi montati in articolatore,
si procedeva alla realizzazione di ceratura
superiore e mascherina per realizzare un
mock up diretto per valutare lunghezza e
forme del gruppo frontale superiore.
Durante la fase di test si riscontrava un ce-
dimento dei canini inferiori a testimoniare
un eccessivo carico nelle lateralità. Questa
indicazione é stata utile per modificare la
ceratura che avrebbe guidato la realizza-
zione del manufatto definitivo. Nel secondo
appuntamento si eseguiva prova delle fac-
cette inferiori e del mock up superiore. Veni-
vano registrate le modifiche da apportare e
rimontati i provvisori. Alla seduta seguente
si procedeva alla cementazione della inte-
ra arcata inferiore sotto diga e alla presa
dell’impronta della nuova arcata e cera di
centrica per cross mounting (Figg. da 18 a
26).

A questo punto, alla luce delle informazioni
ottenute dai mock up, si completava il caso
con la realizzazione e successiva cementa-
zione sempre sotto diga di 8 faccette supe-
riori (Figg. da 27 a 33).
w Risultati
A distanza di 2 mesi si registrava una
scheggiatura del 2.3 risolto con passaggio
di gommini per ceramica ad ammorbidirne
gli spigoli. Probabilmente la guida canina
era ancora troppo ripida in quel punto.
Nessuna dolenzia delle ATM è risultata alla
rivalutazione clinica, il paziente riferisce
stato di comfort. Vi è un ripristino di una
corretta dinamica masticatoria, nessun ele-
mento devitalizzato, adesione dei manufatti
quasi interamente su smalto.

w Considerazioni
La realizzazione di veneerlay in ceramica
mordenzabile a spessori ridotti associa-
ta alla preparazione minimale degli ele-
menti dentali offre una serie di vantaggi
che a nostro avviso non sono confrontabi-
li con una minor resistenza del manufatto
[13,14,15,16]. Questi sono:
1) Adesione su smalto. Questo substrato è
ormai acclarato essere il migliore per l’ade-
sione
2) Mantenimento della struttura dentale.
Con minor rischi di sensibilità post tratta-
mento e minori costi biologici
3) Maggior accettabilità da parte del pa-
ziente. Conseguenza del punto precedente
risulta un elemento fondamentale se si vuole
intervenire in quelle dentature ancora non
così compromesse, in cui la spinta motiva-
zionale del paziente spesso cade sotto la
comunicazione di dover mettere mano alla
fresa per preparare i suoi denti
4) Maggior facilità dei passaggi tecnici. Sia
la presa di impronta che la preparazione
degli elementi risulta più snella e meno pe-
sante per operatore e paziente
5) Provvisorizzazione più serena. Il non aver
preparato gli elementi fa sì che l’eventuale
decementazione o frattura del provvisori la-
sci il paziente nelle condizioni di partenza
senza rischi di ipersensibilità o infiltrazioni.
Come elementi sfavorevoli possiamo elen-
care:
1) Aumentato rischio di frattura dei manu-
fatti: un tale rischio si riduce se l’occlusione
viene programmata in maniera funzionale
eliminando contatti bilancianti e precontatti
in centrica, presupposto questo delle riabi-
litazioni estese. Inoltre l’adesione su smalto
si associa ad un minor rischio di frattura
della ceramica. Infine, non avendo sacri-
ficato struttura dentale, un tale evento non
avrebbe ripercussioni negative sul dente e
potrà essere gestito direttamente in studio
con opportuni accorgimenti.
2) Difficile cementazione. Effettivamente la
cementazione diventa un momento critico,
considerando l’esiguità dello spessore delle
ceramiche e la conseguente fragilità. Inoltre,
l’assenza di un riferimento costituito dalla
spalla di preparazione può portare a errori
di posizionamento. Un protocollo scrupolo-
so che coinvolga l’assistente alla poltrona in
ogni suo passaggio e che prenda il tempo
giusto ridurrà al minimo il rischio di frattura
e malposizionamento.

Per concludere: adottare un approccio ultra-
conservativo completamente additional in
quei casi di perdita di sostanza per abrasio-
ne o erosione soprattutto nelle fasi iniziali in
cui ancora molto smalto risulta presente e
la struttura dentale non risulta troppo com-
promessa rappresenta la scelta più sensata,
considerando che il rapporto rischi/benefici
é nettamente a favore degli ultimi (Figg. 34
e 35).
