  • #4 The Logic Model is an evidence-based behavior change modelused to understand the specific needs and motivations of people in the different stages of change. We use it as a segmentation strategy to tailor content to people's motivation levels, developing content and tools that are effective for each of the stages of change. The logic model was developed through rigorous methodology by researchers at Cardiff University. They conducted a comprehensive review of all published behavior change research (for 5 chronic conditions). They reduced it to the most reliable studies and then identified the techniques that led to significant behavior change. It’s not a system that is proprietary to Healthwise, but is something that has been developed by people across this area of expertise. This structured model will allow us to map or translate our information so that it maps with their systems and frameworks (such as if they are using PAM). People are receptive to (engageable) different types of information in each stage. If the information they receive doesn’t meet their needs – isn’t relevant to them at that point in time – they won’t take it in. For example, if someone has just become aware of a health issue, they won’t be receptive to making plans. It’s not what’s most important to them at this moment. In the same way, someone who is in the sustainability stage could be turned off by receiving basic informative information that is more relevant to the awareness stage. When the health education is matched to the stage a person is in, they’ll respond with “This is exactly what I needed!” When able to match health education to a person’s stage of change, care managers and health coaches will be more effective. Stages: Awareness Learning Motivation Behavior Sustainability 
  • #5 Because it is such a finite value set, we should be able to register the Logic Model stages with an existing code system, such as LOINC.
  • #7 There is significant movement around care plans, both within HL7 and in the S & I Framework at the ONC. Can we do something now to start introducing Care Plan use cases to Infobutton? This could be as simple as adding the notion of problems, risks, goals, and interventions to the task value set.
