Introduction
Hello and welcome to the respiratory module. I will be accompanying you through
examination of the respiratory system in a cooperative child. Remember you must
always adjust your approach and be flexible when examining a child, taking into
consideration their age, personality and how well they are.
A complete examination of the respiratory system must always include an examination
of the ears, nose and throat.
Acute Assessment
Your first encounter with a patient allows you to make some general observations. Most
essential in immediate observation is the acute assessment of the child. This
information determines the pace, the focus and the thoroughness of the exam. In
emergency situations, an acute assessment may require direct medical intervention
prior to the commencement of any further historical or physical investigations. If the
child appears well you will likely have time to complete a thorough history and physical
examination. Keep in mind that much of the diagnosis of the respiratory problem in a
child is formed through your observations. In your initial observations you should obtain
a general impression of the state of the child’s health, observe the nature and effort of
his or her breathing during both inspiration and expiration. Also observe any signs of
respiratory problems, noisy breathing, the quality of any cough and the allergic salute or
shiners.
It is also important to make a quick assessment of the parent accompanying the child.
You can gain valuable information and build rapport by recognizing the emotional state
of the parent.
History
Let’s take a moment to briefly outline the major respiratory symptoms and historical
points to be considered before conducting a physical exam. Age is an important factor
when considering the common symptoms of childhood respiratory disease. Important
presenting features of respiratory illness in children include cough, sputum, hemoptysis,
dyspnea, chest pain, wheeze, and other forms of noisy breathing. Other symptoms that
may be of importance include hyperventilation, eczema, exanthems, and symptoms
associated with gastrointestinal disease. A history of recurrent or unusual respiratory
infections, middle ear infections, or a previous history of croup can be clinically

-القصة العائلية - الكحول
الضغط الشرياني غالبا لعرضي , وفي كثير من الحيان يكشف من خلل
اختلطااته....وهذا يؤكد على أهمية الفحص الدوري للمرضى وخاصة
اصحاب الخطورة العالية .

دون وجود أمراض مرافقة 15 % ياعانون من
اعتلل شبكية بارتفاع التوتر الشريااني
الموجودات العينية ساهمت في تشخيص ارتفاع
- ضغط شريااني غير مشخص مسبقا بنسبة 47

10. أول"--اعتلل الشبكية بفرط التوتر الشرياني
إن استجابة الوعية الشبكية والتغيرات الشبكية المشاهدة
في ارتفاع الضغط تكون متنوعة , وتعتمد على عدة عوامل
و إن أكثر هذه العوامل أهمية هي درجة ارتفاع الضغط و
الحالة البدئية للوعية الشبكية .
كما أن المراض المرافقة مثل السكري والضطرابات
الكلوياة واضطرابات النسج الضامة تلعب دورا" في شدة
الموجودات السريارياة .
11. اللية المراضية :
-باعتلل الشبكية بارتفاع الضغط تحدث اذية على مستوى الوعية الدقيقة
- تبدأ اللية المراضية بفشل التنظيم الذاتي لوعية الشبكية, مما يؤدي الى تخرب
بخليا الندوتيليوم مع تخرب الحاجز الدموي الشبكي, وزيادة الضغط عبر جدار
الشعيرات الدموية مع تثخن جدارها وزيادة نفوذيتها , مما يؤدي الى تسريب
السوائل –الدم -الجزيئات الكبيرة من لمعة الوعاء الى طبقات الشبكية
بالضافة الى حدوث انسداد بشرينات الشبكية النتهائية ممايؤدي لحتشاء في طبقة
اللياف العصبية .
17. إن الوعية المصابة بالتصلب تنقص لديها المقوية العضلية وتميل
للتوسع بزيادة الضغط داخل لمعة الوعاء
أمةا الوعية الغير مةصابة بالتصلب فتظهر تضيق بسبب سلمةة الجدر
العضلية وفعالية استجابة التنظيم الذاتي
لهذا السبب فإننا نجد التقبض الوعائي الصرف استجابة لفرط
الضغط فقط عند الشباب حيث أن أوعيتهم لم تصاب بالتصلب
الشرياني بعد.
18. نزوف الشبكية :
• النزوف ضمن الطبقات الكثثر سطحية مةن الشبكية الداخلية تملك مةظهر
النزوف اللهبية , لنها تسير على طول مةحاور طبقة اللياف العصبية
• أمةا النزوف العمق في الشبكية فتكون نقطية أوبقعية
• إن النزوف اللهبية السطحية أكثثر شيوعا مةن العميقة

21. إن النتحات القاسية وبقع القطن والصوف والوذمةة الشبكية هم تظاهرات
إضافية للطور النتحي مةن اعتلل الشبكية بفرط الضغط ويشيرون إلى
مةرحلة أكثثر خطورة مةن المرض وهذا يتم نتيجة الرشح الوعائي
22. &-النتحات القاسية تنتج عن نضح البلسما مةن جدران الشعيرات الدمةوية
، وهي تتألف مةن دسم وكثولسترول , مةعطية إياها المنظر الشمعي
الصففر المميز, وهذه النتحات عادة تتوضع في القطب الخلفي وتتخذ
أشكال" تعكس مةصدر التسريب مةثل النتحة الحلقية.
23. إن الوذمةة الشبكية المزمةنة تؤدي لتوضع النتحة القاسية حول الحفرة
مةعطية شكل نجمي في اللطخة Henle layer في طبقة fovea
. Macular star
24. بقع القطن والصوف هي عبارة عن مةناطق إقفار شبكي داخلي
حاد بسبب انسداد الشرينات النتهائية وهي تملك مةظهر أبيض زغبي
بحدود غير مةنتظمة وعادة تتواجد في القطب الخلفي. Fluffy
تتوضع هذه النتحات ضمن طبقة اللياف العصبية وغالبا" مةا تشمل
الخليا العقدية المتوضعة تحت هذه الطبقة وكثذلك الطبقة النووية
الداخلية بينما لتشمل طبقات الشبكية العمق .
التصوير بالفلورسين يظهر عدم وجود تروية لبقع القطن والصوف
وللشعيرات الدمةوية المجاورة .
بقع القطن والصوف تزول بشكل نموذجي بتنظير قعر العين خلل
6-4 )أسبوع تاركثة خلل مةوافق في طبقة اللياف العصبية . )
25. التصلب الشرياني :
جدار الشرين في الحالة الطبيعية غير مةرئي ويظهر فقط كثعمود مةن
الكريات الحمراء مةع انعكاس مةركثزي للضوء على مةنظار قعر العين .
26. في اعتلل الشبكية بارتفاع التوتر الشرياني يحدث تنكس هياليني في
طبقة المحددة الباطنة مع ضخامة بطبقة اللياف المتوسطة مع فرط تنسج
بالبطانة , ممايؤدي الى تسمك جدار الشرين..
إن تسمك الجدار الوعائي للشرين يؤدي لظهور تغيرات التصالب
الشرياني الوريدي لن الشرينات والوريدات تشترك ضمن لفافة غمدية
واحدة في مةنطقة التصالب فكلما زادت سماكثة جدار الشرين تناقص
قطر الوريد .
32. الشريان التصلب بين وبوضوح التفريق يتم أن أجل ومن
مرا الن سنذكر الضغط بفرط الشبكية اعتلل وبين ي

لكل منهما : Scheie الشرياني و سنأخذ أبسطها وهو تصنيف

1 تضيق شريني يكشف بصعوبة تغيرات المنعكس الضوئي يكشف
بصعوبة
2 تضيق شريني واضح مع شذوذات تغيرات واضحة لزيادة المنعكس

34. فيما يلي صور تظهر تشارك مظاهر الضغط الدموي مع التصلب الشرياني
من التوتر الشرياني مع تقبضات معممة , نزوف، نتحات IV مرحلة
من التصلب الشرياني ووجود علمة سلك الفضة IV وذمة قرص بصري + مرحلة
35. من التصلب الشرياني متوافقة مع II من التوتر الشرياني + مرحلة IV مرحلة
هجمات تفاقم ملحوظ للتوتر الشرياني
36. من التصلب الشرياني IV من التوتر الشرياني + مرحلة II مرحلة
متوافقة مع ضغط دموي مضبوط
37. اختلطاات اعتلل الشبكية بفرط الضغط الدموي:
-1 انسداد وريد شبكي مركزي أو فرعي
-2 انسداد شريان شبكي مركزي أو فرعي
وأمهات دم مجهرية . Macroaneurysms -3 أمهات دم كبيرة
-4 اعتلل عصب بصري أمامي اقفاري
-5 شلل عصب عيني محرك.
-6 التأثير سلبا" على اعتلل الشبكية السكري.
-7 أغشية أمام الشبكية .
-8 توعي شبكي.
-9 نزف زجاجي
-10 وذمة لطخة كيسية
43. يحتاج IV أو III إن ارتفاع الضغط المترافق باعتلل شبكية مرحلة
إلى معالجة عاجلة لكن المعالجة بالطريق غير الفموي بشكل عام، غير مستطبة
بسبب مخاطر نقص الضغط السريع ومخاطر حدوث احتشاء
دماغي أو احتشاء شبكية العين وذلك ناتج عن الخلل السريع لعملية التنظيم
الذاتي .
4 أيام - إن المعالجة الفموية التي تنقص الضغط الدموي التدريجي على مدى 3
كافية عادة.
44. ثانيا": اعتـلل المشـيمـية بفـرط التوتر الشرياني
إن المشيمية حساسة جدا لتغيرات الضغط الدموي التي تؤثر وبشكل غير مباشر على
والشبكية الحسية المغطيتان لها . RPE الـ
استجابتها الفيزيولوجية لتغيرات الضغط الدموي مختلفة جدا" عن استجابة السرير
الوعائي الشبكي .
المشيمية تتلقى تعصيب ودي وهي حساسة للعوامل المقبضة الوعائية الدورانية مثل
. Vasopressin و Adrenaline و Angiotensin II
هذه العوامل والتنبيهات العصبية يمكنها خلق تقبض لوعية و شعيرات المشيمية مؤدية
والحاجز الدموي الشبكي الخارجي. RPE لقفار مشيمي بؤري وقد يتأثر بهذا كل من الـ
45. المظاهر السريرية لعتلل المشيمية بفرط الضغط :
تحدث عادة عند المرضى الشباب الذين يعانون من فرط ضغط حاد مثل مرضى
ومرضى ورم القواتم Eclampsia والرجاج Preeclampsia قبل الرجاج
أو الضغط المتسارع غير المضبوط . Pheochromocytoma
وهذه المظاهر هي :
RPE وهما عبارة عن احتشاءات في الـ Siegrist وخطوط Elschnig بقع
والمشيمية.
نتحات تحت شبكية .
انفصال شبكي مصلي.
المزمن). )RPE زوال اصطباغ الـ
تصلب المشيمية .
46. إن التغيرات المباشرة على السرير الوعائي المشيمي صعب اكتشافه سريريا"بتنظير
قعر العين لكننا نلحظ موجودات متعددة من تغيرات في الشبكية نتيجة للستجابة
المشيمية لفرط الضغط الدموي .
متطابقة مع نقص التروية RPE هي احتشاءات اقفارية للـ Elschnig إن بقع الـ
المشيمي تحتها.
47. كآفات بؤرية Elschnig تبدو بقع
تحت الشبكية , تكون صفراء في
المرحلة الحادة ومع مرور الوقت مصطبغة
.
أما الحتشاءات االقفارية التي تظهر عند
خط الستواء فتملك مظهر خطي ويشار لها
.Siegrist بخطوط
48. متوذما" فإن الحاجز الدموي الشبكي يتحطم مؤديا"لتسريب RPE عندما يصبح الـ
