1) There are 5 steps for filling a Freedom of Information/Right to Information (FOI/RTI) request which includes filling out an application form providing personal details like name, address, contact information and details of the public body and record being requested. 2) The application form also requires the applicant to provide the purpose, nature of the record being requested and include a declaration that the application fee has been paid and the information will only be used for the specified purpose. 3) The applicant must sign and provide their name at the end of the application form.
