This document appears to be the results of a survey about people's use of the internet to access health information. It includes questions about whether people have used the internet for health information, how often they do so, how many hours they spend searching for medical information online per day, whether they have followed up with doctors about online diagnoses, if they have made medical decisions based on online symptom checkers, their criteria for choosing health websites, and their age. The questions received between 115-118 responses.
