OBJECTIVE
A position as a community provider of occupational therapy services in the home-care setting
SKILLS
I have worked in rehabilitation departments in skilled nursing facilities in Brooklyn, Queens, Nassau and
Suffolk counties since August 2010. My responsibilities have included the following:
- Perform initial screens and evaluations of patient's daily living skills including administering diagnostic
assessments of function.
- Develop treatment plans/care plans with measurable, functional objectives
- Provide appropriate therapeutic interventions in accordance with physician orders to improve/restore strength,
coordination, range-of motion and function or teach/implement compensatory strategies and techniques to
increase performance of ADLs.
- Participate in multidisciplinary care planning for successful discharge outcomes.
- Complete all necessary clinical documentation including evaluations, progress notes, discharge summaries in
a timely manner and in accordance with reimbursement guidelines;
- Supervise and instruct COTAs & therapy aides to effectively provide occupational therapy treatment
programs.
- Recommend and train patients and caregivers in the correct use of adaptive equipment and DME to improve
safe performance of ADLs in the home and community environments.
- Responsible for issuing w/c seating and positioning devices to promote correct postural alignment and
improve biomechanical function.
- Communicate patient progress and status to other health care personnel and patient family members.
- Assess and fit upper extremity splints/orthotics to maintain joint integrity and bilateral hand use
- Provide patient and caregiver education including functional transfer training and w/c mobility training
