This Practice Direction provides guidance on the contents of health reports required for adoption orders or section 84 orders. The health reports must contain detailed medical histories of the child, including any illnesses, disabilities, treatments, and the parents' medical histories. They must also include a full medical examination and history of the prospective adoptive parent(s), covering family medical histories, the applicant's medical history, examinations, and current health status. The purpose is to give the court's medical advisor a full picture of the child's and applicant's health to advise on the implications of the proposed adoption.
