Once you have a basic plan to protect yourself and your family in the event of incapacity or death, you have a foundation upon which to build more sophisticated planning, if required.

Diane Richards has three major routes to take to work. She can take Tennessee Street the entire way, she can take several back streets to work, or she can use the expressway. The traffic patterns are very complex, however. Under good conditions, Tennessee Street is the fastest route. When Tennessee is congested, one of the other routes is usually preferable. Over the past two months, Diane has tried each route several times under different traffic conditions. This information is summarized in minutes of travel time to work in the following table:

This is the letter that was sent by pastors asking President Obama to make a religious exception in the law concerning the hiring of LGBT individuals by religious organizations. Under current plans, President Obama would require religious organizations to hire members of the LGBT community in order to compete for dollars which have traditionally been delivered to religious organizations to help them serve the community and public. Will Obama respond favorably? Or will he simply impose his views without regard to closely held religious practices? We will see.
On Monday, June 5, 2017, the U.S. Supreme Court rendered a much-awaited decision in Advocate Health Care Network v. Stapleton in favor of church plan sponsors that are “principal purpose organizations.” This ruling is significant because it overturns prior contrary rulings affecting church plans sponsored by church-related organizations in the Third, Seventh, and Ninth Circuits.

How To Partner Effectively With Faith Based Organizations
Fbcm Consent Form Members 3 21 10
1. FAITH BIBLICAL COUNSELING MINISTRIES
CONSENT FORM AND AGREEMENT
[Member(s) of Faith Baptist Church]
I/we desire to receive counseling and ministry from Faith Biblical Counseling Ministries, Lafayette, Indiana (“FBCM”). I/we make the following acknowledgements and agreements and give the following consents:
The counseling will be by a counselor who is trained in biblical counseling who may or may not have other certifications or degrees.
A basic principle of biblical counseling is that the Bible is sufficient to understand how to live a God-pleasing life.
The counseling I/we will receive will be based upon scriptural principles rather than principles of psychology or psychiatry.
The counseling I/we will receive will be based on the conclusions that the Bible is inerrant and authoritative, and contains God’s answers to the issues of life, conduct and relationships.
The counseling I/we will receive will reflect the counselor’s understanding of the Bible and how the principles of scripture apply to the problems, questions or issues involved.
Because FBCM is also a training ministry, trainees (i.e. those being trained in biblical counseling) may be present in the counseling sessions and abide by the confidentiality statement articulated in #9 below.
FBCM wants to be a blessing to the community by providing facilities and counselors who volunteer their time and services. While the counseling is free there is a $25.00 cancellation fee payable in cash at the first counseling session. If an appointment is cancelled less than 72 hours before the scheduled time for counseling, the counselee will forfeit the cancellation fee and he or she must submit another $25.00 in cash before continuing in counseling. If all sessions are attended or timely cancelled, the cancellation fee will be returned at the final counseling session.
If a dispute arises concerning the counseling of FBCM, or between a counselee and the counselor involved, or with Faith Baptist Church (“the dispute”), there shall be no lawsuit initiated and the claim cannot be filed in any state or federal civil court. Instead, the dispute must be resolved as follows:
The parties acknowledge that they are all believers in Jesus Christ, and are members of same local body of Christ, namely Faith Baptist Church at Lafayette, Indiana. The parties hereby exercise their election, as provided in Indiana’s Uniform Arbitration Act (I.C. § 34-57-2-1 et seq.) to submit any dispute or claim to Christian resolution. The parties acknowledge that it is very important to avoid bringing reproach upon the name of the Lord Jesus Christ, or upon FBCM or Faith Baptist Church. The counselee(s) further acknowledge that they do not want to be a cause of division or disunity among the brethren, and that they choose to be bound by the biblical procedures set out in First Corinthians Chapter 6 and Matthew Chapter 18. Therefore, counselee(s) hereby promise and commit that if there is such a dispute or claim, counselee(s) will not file a legal action in the civil courts, but instead, they will present the dispute to the pastors and leaders of Faith Baptist Church for Christian resolution. The counselee(s) acknowledge that under God’s guidance, in that context, the pastors and leaders of Faith Baptist Church are wise men who are able to judge between the brethren, and they are therefore able to decide and judge between the parties in the case of such dispute or claim. Furthermore, the parties agree that the pastors and leaders of Faith Baptist Church shall have unlimited discretion to decide the procedures and rules for investigating and deciding such dispute, applying biblical principles, and any other principles that they deem to be fair and just. The parties acknowledge and agree that the decision and judgment of the pastors and leaders will be final, non-appealable, and that the parties will accept and submit to such decision, and will apply all effort and diligence to maintaining the peace and unity of Faith Baptist Church, including confession, forgiveness, repentance, restoration, and reconciliation
Counseling at FBCM, including statements made during counseling, shall remain confidential, with the following qualifications and exceptions:
Counselors shall be free to discuss counseling sessions and cases with other FBCM counselors and the pastors of the Faith Baptist Church, Lafayette, Indiana in order to gain the benefit of additional insight and input;
Counselors and pastors shall be entitled to seek a confidential legal opinion or advice from an attorney when it is deemed appropriate and helpful;
If any FBCM policy concerning the reporting of child abuse or child neglect, or the reporting of elder abuse or elder neglect, mandates a report to Child Protective Services or other authorities in compliance with the laws of the State of Indiana, then such report will be made;
If a suicide risk is indicated, the counselors or pastors may seek necessary help and make whatever reports or disclosures as they deem to be proper and necessary;
If a counselee indicates an intention to commit a crime, such intention may be reported and disclosed to the proper authorities;
I/WE HAVE READ AND UNDERSTAND THE ABOVE PROVISIONS, AND I/WE UNDERSTAND THEM AND I/WE HEREBY ACKNOWLEDGE AND AGREE TO ALL OF THE ABOVE TERMS, INCLUDING, BUT NOT LIMITED TO, THE CANCELLATION POLICY, WAIVER OF SEEKING REDRESS (LAWSUIT) IN THE CIVIL COURT SYSTEMS, AND AGREEING THAT ANY AND ALL DISPUTES BE RESOLVED THROUGH CHRISTIAN RESOLUTION AS DETAILED ABOVE.
____________________________Date:____________
(1) Counselee Signature
____________________________
(1) Counselee Printed Name
____________________________Date:____________
(2) Counselee Signature
____________________________
(2) Counselee Printed Name
____________________________Date:____________
(3) Counselee Signature
____________________________
(3) Counselee Printed Name
FAITH BIBLICAL COUNSELING MINISTRIES
By:______________________________Date:____________
Signature
______________________________
Printed Name
______________________________
Title/Position
rmwFILENAME R:+LAFAYETTEaith Baptistounselingaith Biblical Counseling Ministries Consent Form.member.doc
[Revised 3/2010]
