There was one eligible couple in the family. For family planning they have
opted for the natural and barrier method of contraception i.e. abstinence ,
condoms . They have 1 child (boy) They are not planning for any child further .
Recommendations : if the couple is not further planning for another kid then
they can opt for the terminal methods of pregnancy namely tubectomy or
vasectomy.

past history - Oedema all over body in childhood , protein in urine , treatment for 6
years.
Personal history - no history of any drug intake
Diet. - Mixed diet
Addiction - no addiction
17. Signs of Puberty :

- Beard / moustache no significant growth of any beard or
moustache
- Spurt in height no significant growth spurt in height
Immunization :
- TT 1 ( at 10 years ) vaccinated

18. MENTAL AND PSYCHOLOGICAL ASSESSMENT
He does not feel sad or loss of interest in routine activities.
No problem in concentrating in work or any feeling of low self
esteem or low self confidence.
No feeling of guilt or unworthiness.
Does not have bleak or pessimistic view of future or ideas or acts of
self flagellation or suicide.
No apparent disturbance in sleep at night or reduction in appetite.
21. RECOMMENDATIONS :
For energy deficiency he can start taking fruits like bananas and
other dairy products in his diet.
He can also increase intake of ghee, potatoes and eggs in his
diet.
For protein excess he does not need to cut off anything from his
diet since he’s a growing child slight excess in protein is tolerable
and variation can also be seen according to everyday diet.
For iron deficiency he can increase intake of green leafy
vegetables, jaggery, peas, bananas, apples , eggs, dates,
chicken, etc.
The mother can be advised to cook food in iron utensils.
For calcium deficiency he can increase intake of milk products in
his diet for example curd, paneer and green leafy vegetables, etc.
