This document summarizes data on surgical incidents from different departments. It shows that there were 620 total procedures, with 30 incidents occurring, meaning 5% of procedures had an incident. Neurological and Orthopedics departments had the most incidents with 7 and 10 respectively. Cardiac had the least with 1 incident. The most common types of incidents were sponge counts and instrument counts, each with 5 incidents.
