This document contains a questionnaire to diagnose customer perceptions of an educational institution called "love corner". The questionnaire contains 14 questions regarding the institution's mission and vision, communication with teachers, health and safety standards, educational methods, advertising image, discipline, opening hours, child's learning development, educational activities, overall service, facilities, play area, and prices. Respondents are asked to rate various aspects on a scale of 1 to 5 or answer yes/no questions.
