This document has important information for students from high school to take the Nurse Aide Training Program at the College. You should follow all of the deadlines as it is critical for you for acceptance. Please read all paperwork you received at the information session very carefully.

Important Dates to Remember/Requirements:
October 19, 2012- All Paperwork due to Sharvette Law Philmon- Nurse Aide Program Coordinator
at the DCCC College Office at the Pennock’s Bridge Campus. Please notify the staff there to place
your packet in the Assistant Dean’s mailbox.
In this packet, we need all the forms that require a signature completed along with the following
documents:
Criminal background Check - Students to obtain this at the following
website https://epatch.state.pa.us/Home.jsp - Please see the sample document
you received with the Pennsylvania State Seal. The document you print should look
exactly like this.
Medical Verification Form- completely filled out, signed and dated. (With the 2 step PPD
completed Physical Exam- within the last year with 2 TB test must be at least 7 days apart.
Pennsylvania Residency Verification Form
A copy of your Medical Insurance Card - (This card must have your name on it).

Academic Information/Process:
Students will complete the course work during the school year 2012 to 2013 with a start
date of November 7, 2012.
Tuition: Last Day to Pay - October 30, 2012
Student’s enrolled in the TCHS Health Occupations/Careers benefit from the partnership
the school has with Delaware County Community College. Students will pay $295.00 for the
course plus a $5.00 processing fee for a total of $300.00 (Acceptable forms of payment
include: Money Order or Credit Card).

Other Important Information:
Attendance is Mandatory- any time missed time must be made up and the student will not
be able to test with the American Red Cross at the end of the program.
Students must wear their TCHS scrubs to class and to clinical in the summer of 2013.
2. The program requires a 75% pass rate on tests for the student to progress to clinical. Each
test consists of approximately 10 questions and there are 16 exams.
Students who successfully complete the program will be given a certificate of completion
from the college after completing the summer clinical portion of the course. Please keep
your certificate as this is your proof of completing the program in Pennsylvania.
Materials:
Students will receive a custom binder from the College at the beginning of the course.
Textbooks - Mosby’s 8th ed- Students are currently using in the high school.

Students are responsible for transportation to and from the clinical site.
American Red Cross testing:
Cost -$102.00 This money is due on the last day of clinical and can only be paid by money
order.
Any time missed in Classroom, Sim Lab or Clinical must be made up or the student will not
be able to take the American Red Cross Testing.
After each class day, students are required to review their schedules and record their study time
in their planners, calendars, or telephone. The faculty verify that students are documenting
their study times and dates at home. Students should take their work schedules and other
responsibilities when planning their study time.
