department allow LQGs and TSDs to file reports
annually if reports were filed electronically.

past several years

3. Registering to use the system
• Obtain a username from the Citizen Application
Gateway (CAG). It will begin nr.
• Complete the “Registration for Hazardous Waste
Generator E-Reporting” (MO 780-2531)
• Notarization only required to add a certifier
• Two signatures must be notarized, the certifier’s and
the “generator’s.
• The same person may sign as both the certifier and
generator

• Certifiers must send proof of identity (such as
an unaltered copy of a driver’s license) and
proof of ownership (copy of paperwork filed
with the Secretary of State) or association
(short letter on letterhead stating certifier’s
position with the company) with the
participating generator.
• Send in all three pages of the form, even when
not adding a certifier.
5. Common Rejection Reasons
• Submission did not have a valid
CAG username.
• Certifiers did not send proof of
identity or association.
6. You will know you are registered….
• When you receive a welcome email from the
system.
• The welcome email for a certifier will contain
an initial PIN (5 digits). The first time he logs
into the system he will be taken to the PIN
management screen.
