Colorectal cancer (CRC) has potential to spread within the peritoneal cavity, and this transcoelomic
dissemination is termed “peritoneal metastases” (PM).The aim of this article was to summarise the current
evidence regarding CRC patients at high risk of PM. Colorectal cancer is the second most common cause of cancer
death in the UK. Prompt investigation of suspicious symptoms is important, but there is increasing evidence that
screening for the disease can produce significant reductions in mortality.High quality surgery is of paramount
importance in achieving good outcomes, particularly in rectal cancer, but adjuvant radiotherapy and chemotherapy
have important parts to play. The treatment of advanced disease is still essentially palliative, although surgery for
limited hepatic metastases may be curative in a small proportion of patients.

2. Specialty recommendations for England, Wales and Northern Ireland. In 2010 The Association of Surgeons of Great Britain and Ireland (ASGBI) recommended a consultant workforce ratio of 1:25,000 population and an overall maintenance of consultant surgeon numbers. These recommendations will vary depending on each sub-specialty.

Enhanced Recovery (ERAS) in Colorectal Surgery is a relatively novel concept in patient care. It involves a multidisciplinary team approach (surgeons, anesthetists, ERAS nurse, nutritionist, physiotherapist, pain team, hospital administration and patient motivation) comprising of certain key aspects in the pre, intra and post-operative settings.

By utilizing evidence based practice enhanced recovery after
surgery (ERAS) protocols implement several steps along the care pathway to help minimise the surgical stress response caused from surgical insult. Radical Cystectomy is associated with the highest morbidity of all urological procedures [1]; with extended length of hospital stay and high complication rates reported post operatively [1-
2]. In 2013, following a literature review the ERAS society published guidelines detailing 22 ERAS items for patients undergoing radical cystectomy.
This highly interactive and multidisciplinary workshop is
designed to increase the practical knowledge and skills critical
to establishing and maintaining a successful program for
the care of patients with peritoneal surface malignancies.
The first day of the course will focus on a real time HIPEC
surgery procedure streamed live to the conference venue,
providing unique learning opportunities. National experts will
share their knowledge and experience essential to creating
a successful and comprehensive peritoneal malignancy
program for gastrointestinal and gynecologic malignancies.

After opening my practice in 1994, I developed a strong practice and reputation for my
interests in minimally invasive surgery and my work in cancer surgery. I helped develop
the Thoracic Oncology Group and had one of the highest attendance rates at our
multidisciplinary tumor boards. I’m known for my thorough evaluation of patients and
my surgical results. I was the busiest minimally invasive surgeon in the region and
performed the majority of advanced procedures.
RECENT ACCOMPLISHMENTS
In 2007, I was hired by CA Dean Memorial Hospital in Greenville, Maine to develop a
surgical program. While there, I initiated OR protocols, brought the OR into compliance
with regulatory requirements, instituted the SCIP guidelines and increased General
Surgery case volumes from 16 cases per quarter to about 50 per month. I’ve also
expanded the surgical services to include Orthopedics, Gynecology, Urology and
Podiatry and CAD hired 4 surgeons during my tenure. I designed an upgraded PACU and
initiated peri-operative protocols. A colon cancer screening program was developed that

included a data base system to track every patients’ colon cancer risk, the date and results
of their last colonoscopy and a system to ensure that the patients’ next screening would be
scheduled for the appropriate time.
My leadership skills include the ability to improve quality of care, streamline patient
flow, close gaps in regulatory requirements, provide patients with a compassionate and
efficient service, improve documentation and increase the efficiency of a surgery center.
My approach to administrative and process problems is the same as my approach to a
clinical problem; an analysis is done, a diagnosis is made and a treatment is initiated.
Progress is assessed and the intervention is modified until the problem is cured. CA Dean
Memorial Hospital has recently won several state and national awards for quality of care
and is the highest ranking hospital in Maine for patient satisfaction!
In 2011, I brought to maturation an agreement between Tufts University Medical School
and CA Dean Memorial Hospital that designated the hospital as a site for first year
medical student orientation, the second year Competency-based Apprenticeship in
Primary Care [CAP] course and third year clinical clerkship rotations.
In 2015, I was hired by Penobscot Valley Hospital in Lincoln Maine to essentially
reproduce those initiatives and results.
