This document is a pre-entry form for optional singles bowling at the 28th C.R.C. It provides spaces to enter up to four bowlers with their name, gender, and lane assignment. The entry fee is $10 per bowler with a payout of 1 in 15 and $8 going to the prize fund. Handicap is 80% of the difference between the bowler's average and 200. The event is separate from the regular tournament singles. Pre-entry forms and payment are due by January 15, 2012 and can be mailed to the provided address. Lanes will be assigned Saturday morning, noon, and afternoon.

HANDICAP 80% of the difference between average and 200, this event is bowled in conjunction with
the regular Friday and Saturday bowling and is separate from the regular tournament singles event. This IS

You may use this form to enter up to four bowlers, and you may
duplicate this form if necessary. You may pre-enter by mail or enter
anytime before you bowl in the 28th C.R.C.
