Streets To Homes provides outreach and follow up support to end street homelessness. During his 200 hour placement, Christopher worked with the street outreach, housing outreach, assessment and referral centre, and housing follow up teams. He accompanied street outreach workers and learned about engagement skills, safety protocols, and connecting with homeless individuals. Christopher also experienced the interconnectedness of the programs by following a recently housed client from housing outreach to housing follow up. He demonstrated strong work ethic, listening skills, advocacy, and connecting his client to community resources. Christopher is hardworking, resourceful, and highly motivated to succeed in social work having experience working with the homeless in the Philippines.

Social workers- are sometimes referred to as case managers. Their main duty is to connect with people to ensure the current situation does not get out of hand. They coordinate social services across multiple agencies and service providers like helping children living with HIV/AIDS, the homeless and orphaned kids. They are also responsible for acting as child rights activists in certain organizations.
Youth counsellors- they offer short-term counselling and case management services to youths in institutions that are set apart for the homeless or also in the streets where they sometimes indulge themselves in substance abuse due to lack of counselling, that’s why these counsellors are of very much assistance to the homeless societies. Youth counsellors assist in crisis intervention, psycho-education and also in the management of cases.
Professional responsibilities
The youth counsellors are responsible for providing counselling services, case management and also advocating for their clients, ensuring that there is a linkage between the youths and the community referral resources. Professional youth counsellors also provide necessary documents required in case of consultation or available programs for the youths by managing the relevant documents. The counsellor provides any necessary statistical information and any information that is necessary for any form of evaluation. They also ensure and provide ongoing outreach to young people.
Social workers, on the other hand, represent their clients by providing secondary consultation to the relevant lawyers and also ensure that there is a healthy relationship between the homeless and relevant agencies and services. The social workers may at times collaborate with other professionals in writing up assessments that meet specified standards and timescales. In cases such as child protection or mental disorder, they are involved in the multidisciplinary teams or any other meetings that the individuals may be involved in. They also ensure that they have the relevant documents available and ensure they are up to date as much as they represent their clients in the relevant meetings. They can also be present in court to give evidence pertaining a case related to their clients.
Knowledge, skills, and attributes.
For the social workers, one should demonstrate strong interpersonal skills such as being in a position to handle challenging or complex personalities. One should also possess high level of writing and communication skills and the ability to handle relevant official documents and keep them confidential, accurate and up to date. The services listed above are utilized by our surrounding community, to those who need it, at any individual’s request. The time frame to try and complete a mission is approximately 6 months, subject to each individual’s particular case. The outside surroundings consist of the complications of problems at hand, the satisfactoriness of the organization which regulates the revenue of admittance and finally the combination of the people being served (California Health and Human Services Agency, 2016).

The rules in the manual hold guidance plans, practices and protocols to be followed by the center (Florida Department of Human Services, 2016). The Notice of Privacy Practices is another set of policies, which is an explanation of how the individual’s medical information is protected and can be utilized by the Culmer Community Action Center or any release that relates to the healthcare, treatment, or procedures. According to the law (Gostin, 2008), for instance; the guide designates the code of behavior to observe when the person who is giving treatment wants to access the data for an assessment for the services be given.

To Whom It May Concern:
Streets to Homes was established in 2005 and it consists of a network of internal and
community services to provide outreach and follow up support in order to end street
homelessness. During his 200 hour placement with Streets to Homes Christopher worked
alongside staff in all aspects of the program which includes Street Outreach, Housing
Outreach, the Assessment and Referral Centre and the Housing Follow-Up team.
Christopher reported directly to me and the staff he worked with reported to me weekly
concerning his activities and learning plan.
He accompanied the Street Outreach workers for six days and learned about the
importance of engagement skills and persistence when identifying and forming
relationships with isolated individuals. Most often these are men who refuse shelter
services and have untreated mental illness often accompanied by substance use. Women
are encountered less frequently. He learned about the safety and communication
protocols used on the team and their role in attending to encampment clearances and
concerned citizen calls as well as daily rounds connecting with homeless individuals on the
street, in parks, alley ways and ravines.
Christopher experienced the interconnectedness of the Streets to Homes programs when
he followed a recently housed client with complex medical issues from Housing Outreach
to the Housing Follow Up team. The first team works with people to find them housing
which involves developing relationships with landlords, learning about the Ontario Works
and Ontario Disability Support Programs shelter allowances, the Toronto Transitional
Housing Allowance, Furniture Bank and the Housing Stabilization Fund. He was a very
motivated learner and he showed initiative in seeking out staff to explain the housing
process to him.

Christopher really shone in his work with his assigned client and demonstrated a strong
work ethic, an ability to listen to and respect his client's wishes, recognize and validate
client strengths and to advocate for his client. He researched and mapped out community
resources, accompanied his client to medical specialist appointments, referred him to
Wheel Trans and connected him to PEACH (Palliative Education and Community Health).
The latter referral required tact and sensitivity and Christopher was a support to both the
community nurse, his Housing Follow Up mentor and the client.
Christopher is hardworking, resourceful and highly motivated to succeed and to work in his
chosen field as a social worker. He brings years of experience working with a homeless
population in the Philippines and he is adapting to working with the homeless population in
Toronto. He is dedicated to providing excellent customer service and to expanding his
knowledge.
