Mrs. E, a 67-year-old woman, was admitted to the geriatric division for management of multiple pressure injuries and complications from a previous stroke. She has a history of hypertension and stroke in 2016 resulting in weakness on her left side. Her pressure injuries have worsened due to lack of proper wound care and immobilization. On examination, she has severe functional impairment requiring full assistance with activities of daily living. She is depressed and at high risk of falls, malnutrition, and infection. The goal is to resolve her psychological issues, prevent worsening of wounds and new injuries, and improve her mobility and quality of life through a home care plan.
