2. Referral to DOH accredited MD (DDE) for admission to
Treatment and Rehabilitation Center
3. Referral to Psychiatrist for clearance if with other mental
health problem/s
4. Referral to DOH retained hospitals for free laboratory
exams (as requirements for admission)

8. Referrals to Mega DATRC
• Referrals for admission started last December 15, 2016
• FREE of charge
• No referral from LGU, NO admission (must undergo screening
at the LGU)
• All referrals from LGU for admission will be facilitated by RO3
(DDAPTP coordinator )
• Referrals can be done through e-mails (with E-signature of
MHO/CHO) (referral form printed in BLUE paper)
• Received referrals from LGU will be acknowledged accordingly
• All required documents must be available before admission
schedule and be presented subsequently

 Referral letter from LGU through the MADAC/ CADAC in
BLUE paper
Medical Certificate (fit to undergo treatment and
rehabilitation)
Laboratory results
Complete Blood Count (CBC)
Chest X-ray
Urinalysis
Pregnancy Test for females
ECG for 45 y/o and above or as needed if with medical condition
HEPA screening test and HIV test for patients with tattoo and
piercing/s

13. Other issues:
• Laboratory exams must be done by DOH accredited
laboratories
• Letter from RO3 to DOH retained hospitals for endorsement of
laboratory services to drug surrenderers referred for
admission to Mega DATRC
• Schedule issued for admission must be followed accordingly
• Court order facilitated by DOH RO3
