Family members of patients at Mease Countryside Facility in Florida have complained about unaddressed concerns and delays in updates from nursing staff about their loved ones. While the hospital is award-winning, issues with customer service between families and nurses exist. A key problem is nurses' inability to positively communicate with and respond promptly to families. Hiring more nurse assistants could help address this by taking on extra work when nurses are busy, preventing stress for all while ensuring families receive timely updates. Practicing Organizational Culture Without a Leader

Small Feet OB/GYN was at one time a robust practice with five physicians, a midwife, and two PAs. The practice had a strong following in the community, was trusted by the many women it had served, and recently began delivering “legacy” babies of patients. Dr. Smith was the founder of the practice and had been the lead physician for many years.
Two competing systems with hospitals only one mile apart had vied for the affiliation with the Small Feet practice. Dr. Smith decided to change her affiliation to the other hospital. As a result, the practice experienced a move that seemed to only strengthen the patient base, and the new space (which was twice as large as the previous office) seemed to suit the new practice well.
The medical staff and CEO of the new hospital supported Dr. Smith’s move for several reasons. First, Dr. Smith, as mentioned, was delivering legacy babies and in the OB/GYN field this speaks to the trust the provider has been able to create and sustain throughout the years. This resulted in lots of community goodwill; that intangible quality is highly sought after in the medical community, yet is so difficult to quantify. Second, the new affiliation of Dr. Smith and her patients would bring positive revenues to the hospital through the move. Finally, the new hospital had a Level 1 trauma center, known for neurological cases, but not for delivering babies. The expansion of labor and delivery with the addition of a seasoned, legacy-delivering physician was a real coup for the hospital to attain.
Dr. Smith became ill and had to go on medical leave for almost a year. During that time the cohesiveness among the other providers suffered. When Dr. Smith came back things were very different. Dr. Smith became suspicious of everyone and had feelings that the staff and other providers were conspiring against her. Her suspicious attitude toward the physicians and staff in her practice led to dysfunctional problems throughout the practice. When Dr. Smith was confronted by the manager, Amy, she became distrustful and suspicious that Amy was conspiring with the other providers in the group against her.
The practice had also gone through some growing pains from a one-physician practice to five. Though originally the physicians worked well together, they now seemed to be less willing to collaborate. The practice also suffered as a result of a manager who had not kept up with the managerial requirements needed to run a midsize practice. For example, staff and provider performance reviews had never been done, the physicians had not established policies and procedures for the practice, there was no.

The Strain of Increasing Patient Populations on Primary Care
One stressor affecting healthcare on a national level is increasing strain that is being placed on primary care providers. In an article published in the journal “Managed Care” titled Primary Care Continues to Feel the Strain (2012), the authors explain that providers in the primary care setting
currently only spend 15 minutes on average with each patient. With the increases in patient needs, these practices simply cannot handle taking on more patients. The demands placed on providers to incorporate preventative care into the treatment of a patient’s current illness or chronic condition are
making it difficult for providers to maintain quality care in such a short visit with a patient.
My current work setting is a primary care office, that provides care to patients on a sliding fee which based on income. We work to serve the uninsured and underinsured population of Eastern Idaho. With five providers currently seeing an average of forty patients a day each, our office is feeling
the strain of increasing patient populations. This strain not only affects the providers, but has added strain to the nursing staff, reception staff and medical billers.
Currently our providers each have their own patient population, where for primary care needs, the patient only sees the one provider. A patient needing refills on his metformin or lisinopril can only be seen by the provider that prescribed that medication currently. The problem this is creating is
the inability to schedule these patients for a follow up before they are out of refills due to a provider's schedule being booked for weeks out.
The solution that has been implemented to aid in this problem is the use of nursing teams. Each provider has his or her own team of nurses. As the lead nurse for my physician, patient phone calls, questions and refill requests come to me first. We also have a care coordinator that helps to
manage patient referrals and making sure imaging appointments are made and reports are uploaded to the patient chart. In chapter 3 of the course text, it is established that successful teams use each member’s strengths to benefit of the team and to the quality of patient care (Marshall & Broome,
2017).

This resulted in lots of community goodwill; that intangible quality is highly sought after in the medical community, yet is so difficult to quantify. Second, the new affiliation of Dr. Smith and her patients would bring positive revenues to the hospital through the move. Finally, the new hospital had a Level 1 trauma center, known for neurological cases, but not for delivering babies. The expansion of labor and delivery with the addition of a seasoned, legacy-delivering physician was a real coup for the hospital to attain.
Dr. Smith became ill and had to go on medical leave for almost a year. During that time the cohesiveness among the other providers suffered. When Dr. Smith came back things were very different. Dr. Smith became suspicious of everyone and had feelings that the staff and other providers were conspiring against her. Her suspicious attitude toward the physicians and staff in her practice led to dysfunctional problems throughout the practice. When Dr. Smith was confronted by the manager, Amy, she became distrustful and suspicious that Amy was conspiring with the other providers in the group against her.
The practice had also gone through some growing pains from a one-physician practice to five. Though originally the physicians worked well together, they now seemed to be less willing to collaborate. The practice also suffered as a result of a manager who had not kept up with the managerial requirements needed to run a midsize practice. The Strain of Increasing Patient Populations on Primary Care
One stressor affecting healthcare on a national level is increasing strain that is being placed on primary care providers. In an article published in the journal “Managed Care” titled Primary Care Continues to Feel the Strain (2012), the authors explain that providers in the primary care setting
currently only spend 15 minutes on average with each patient. With the increases in patient needs, these practices simply cannot handle taking on more patients. The demands placed on providers to incorporate preventative care into the treatment of a patient’s current illness or chronic condition are
making it difficult for providers to maintain quality care in such a short visit with a patient.
My current work setting is a primary care office, that provides care to patients on a sliding fee which based on income. We work to serve the uninsured and underinsured population of Eastern Idaho. With five providers currently seeing an average of forty patients a day each, our office is feeling
the strain of increasing patient populations. This strain not only affects the providers, but has added strain to the nursing staff, reception staff and medical billers.
Currently our providers each have their own patient population, where for primary care needs, the patient only sees the one provider. A patient needing refills on his metformin or lisinopril can only be seen by the provider that prescribed that medication currently. The problem this is creating is
the inability to schedule these patients for a follow up before they are out of refills due to a provider's schedule being booked for weeks out.
The solution that has been implemented to aid in this problem is the use of nursing teams. Each provider has his or her own team of nurses. As the lead nurse for my physician, patient phone calls, questions and refill requests come to me first. We also have a care coordinator that helps to
manage patient referrals and making sure imaging appointments are made and reports are uploaded to the patient chart. In chapter 3 of the course text, it is established that successful teams use each member’s strengths to benefit of the team and to the quality of patient care (Marshall & Broome,
2017).

Staffing issues have always been a major area of concern within the health field. Adequate staffing affects the patient safety, staff morale and finances of the organization. It’s important that organizations take note of concerns related to their staffing and take the necessary steps to ensure that adequate staffing within the units.
The staffing in my organization varies depending on the unit and level of care or acuity of the patients. For starters, patients enter into the Emergency Department and are typically cared for by a nurse who is assigned a team of 3-4 patients. Nurses who work on the medical/ telemetry units are assigned a 1:6 nurse: patient ratio. Nurses who work on the Intermediate Care or Step Down unit is assigned a 1:3 nurse:patient ratio. ICU and CVICU patients are assigned at a 1:2 or 1:1 nurse to patient ratio.
There could be a number of variables that affects the staffing ratio. The shortage in healthcare professionals is one of the greatest variables affecting the staffing ratios. When there is not enough employees on duty, nurses and nurses assistants are forced to work under staff. Patients and families notice when there is a shortage of staff, and they tend to lose their faith in their provider when they’re working short. It’s important that staff understand the importance of managing up the organization and notify their leader if they are feeling overwhelmed instead of venting to the patients.
As it stands now, my organization has some of the lowest nurse to patient ratios which is currently mandated by the organization itself. Our organization takes pride in patient safety, and focuses on safe quality care as a whole. This staffing ratio affects the organizational budget as a great deal of resources is put into recruiting qualified staff as well as using alternative staffing agencies to assist with nursing and support staff coverage.

Problems Faced in the Medical Industries
Executive Summary
It is important for all health care professionals to ensure that due care is provides to them. This requires all qualified medical doctors give patients the time for diagnosis and subsequent treatment. In many health care centers, doctors may be overwhelmed leading to cases where their assistants have to step in and take over their role. In the process, this may compromise the quality of care accorded to the patients and may have significant ramifications legally and otherwise. There are, however, certain measures that can be taken to deal with this problem. One such measure rests on restructuring the system in a manner which the PA’s see them first and do the initial assessment, and then one of the doctors do the final assessment. This might mean cutting down the number of patients seen per day so that the PA’s and Doctors can better focus on taking good care of the ones they do see. This proposal will examine who this system will be implemented in a selected health care center highlighting the benefits that accrue from it.

At Family Healthcare Associates, Inc., physicians and their assistants have always been challenged with huge number of patients in their clinics. The aim of this strategy is to reduce the cost of operation for the clinic due to huge medical expenses available in the market today. Senior management needs to adopt a business like strategy to ensure the clinic remain a float and doctors have enough time to focus on each patient per specific period of time (Fattal, 2011).
Problem
The cost of maintaining Family Healthcare Associates, Inc. has been a challenge due to increase in salaries and financial packages. These increases in financial expenditure by the clinic have made it impossible to manage doctor-patient relationship that most clinics have set standards for. These challenges can pose a threat to the health of many patients who require immediate medical attention on time. The Doctors have such a busy schedule that they do not have the personal one on one relationships with any of their patients that they need to have in order to fully know what is going on with their patients.
One of the most common scenarios where patients and doctors experience these challenges happens during emergencies. Doctors have to abandon their current patients in order to attend to more urgent emergencies of another patient (Gropper, 2009). This isn’t always a big problem in small clinics today, but it does happen from time to time. Sometimes doctors can leave the patient unattended for a long time which can cause health care problems for the patient in the future.
