SHOULDER ARTHROSCOPY SURGERY IN INDIA
3. WHAT IS SHOULDER ARTHROSCOPY SURGERY
Arthroscopy is a procedure used by orthopedic surgeons for
inspecting, diagnosing and repairing issues within a joint.
Shoulder arthroscopy is a procedure that allows visualization,
diagnosis and treatment of several issues within and around the
shoulder joint of a rotator cuff.
The arthroscopy and surgical instruments are thin, and the
surgeon may use very tiny incisions instead of the bigger incision
required for standard open surgery.
Arthroscopic shoulder procedure is used to treat many common
shoulder conditions, including bursitis, tendonitis, arthritis,
rotator tears, labral tears, and the instability of the shoulder.
Shoulder arthroscopy is done with very small incisions, recovery
is faster than with open surgery.

Surgery may be an ideal treatment for many common shoulder problems, particularly those that fail to respond to
conservative therapy. They can range from minimally invasive arthroscopic procedures (in which scope and surgical
instruments are inserted through keyhole incisions in your shoulder).
Arthroscopy for Impingement Syndrome :The arthroscopic procedure used to correct impingement is known as a
subacromial decompression. The aim of the surgery is to increase the space between the rotator cuff and the top of the
shoulder (known as the acromion).
Arthroscopic SLAP Repair: The term SLAP stands for "Superior Labrum Anterior and Posterior." A SLAP tear,
therefore, occurs both in front (anterior) and back (posterior) of the labrum. Arthroscopic surgery may be used to
restore the labrum back to its position at the rim of the shoulder socket. Once repositioned, sutures would secure the
bone to the cartilage. If the damage extends into the biceps tendon, additional surgery may be needed.
Arthroscopy for Shoulder Dislocation: A shoulder dislocation injury occurs when the ball of the shoulder joint
comes out of the socket, Repeated dislocations can lead to severe shoulder damage and require substantial surgery to
keep the joint in place. While there are several different ways to achieve this, the procedures typically involve
repositioning bone around the shoulder to hold the ball more securely in place.

8. CONT…
Arthroscopy for Frozen Shoulder: When a frozen shoulder occurs, the capsule surrounding the shoulder joint
becomes tight and contracted. The goal of surgery is to loosen the contracted tissue to allow the shoulder to move
more freely. This is typically done by cutting the capsule all the way around the ball of the shoulder; this can be
technically challenging given that the space inside the joint will be extremely tight.
Rotator Cuff Repairs: One of the most common surgical procedures for the shoulder is a rotator cuff repair. The
goal of the surgery is to identify the damaged part of the rotator cuff and to clean and reattach any torn or damaged
tendons. Once the healthy tissue is found, the surgeon will use different techniques to restore the tendon without
excessively stretching the remaining tissues.
Acromioclavicular (AC) Joint Repairs: The acromioclavicular joint, commonly known as the AC joint, is the
junction of the end of the clavicle (collarbone) and acromion. There are several problems that can occur at the AC
joint. The first is that it can wear out. This can occur as a result of arthritis, usually at the site of previous trauma. It
can also deteriorate due to a repetitive use injury, such as weightlifting. Instability can also occur at the AC joint,
causing progressive damage to the ligaments that connect the clavicle to the end of the shoulder blade. This can
eventually lead to shoulder separation. While shoulder separation can often be treated nonsurgically, severe cases
may require surgery to repair or reconstruct ligaments that support the end of the clavicle.
9. CONT…
Shoulder Replacement: Shoulder replacement surgery is typically reserved for advanced arthritis of the shoulder
joint but can also be used for complex fractures and other problems that cannot be repaired with other techniques.A
typical shoulder replacement will replace the ball and socket joint with an artificial ball made of metal and a socket
made of plastic. In cases where only the top part of the arm bone (humerus) is broken or the socket of the arthritic
shoulder is still intact, a partial replacement, known as hemiarthroplasty, may suffice. Hemiarthroplasty just replaces
the ball of the shoulder, leaving the socket untouched.Another option is the reverse shoulder replacement. As per its
name, the surgery reverses the location of the ball and socket so that the replaced ball goes where the socket was,
and the replaced socket goes where the ball was. The surgery can provide a mechanical advantage to people who
have a condition called rotator cuff tear arthropathy in which both the labrum and rotator cuff are severely damaged.
Biceps Tendon Surgery: The bicep is a muscle on the front of your arm. At the top end is a tendon, called the long
head of the bicep, that attaches the muscle to the shoulder by traveling through the rotator cuff and affixing itself to
the labrum. This complex relationship makes the long head of the biceps a common target for shoulder problems.
Biceps tendon surgery can be performed in isolation or as part of a rotator cuff repair. There are generally two
approaches used. One is to sever the tendon connection inside the shoulder joint and reattach it outside of the
shoulder joint.
10. WHY THE PROCEDURE IS PERFORMED ?
Arthroscopy may be recommended for these shoulder problems:
A torn or damaged cartilage ring (labrum) or ligaments
Shoulder instability, in which the shoulder joint is loose and slides around too
much or becomes dislocated (slips out of the ball and socket joint)
A torn or damaged biceps tendon
A torn rotator cuff
A bone spur or inflammation around the rotator cuff
Inflammation or damaged lining of the joint, often caused by an illness, such as
rheumatoid arthritis
Arthritis of the end of the clavicle (collarbone)
Loose tissue that needs to be removed
Shoulder impingement syndrome, to make more room for the shoulder to move
around
11. HOW DO YOU PREPARE FOR SHOULDER ARTHROSCOPY SURGERY?
Tell your health care provider what medicines you are taking. This includes medicines,
supplements, or herbs you bought without a prescription.
During the 2 weeks before your surgery:
You may be asked to temporarily stop taking blood thinners. These include aspirin, ibuprofen
(Advil, Motrin), naproxen (Naprosyn, Aleve), and other medicines.
If you have diabetes, heart disease, or other medical conditions, your surgeon may ask you to
see your doctor who treats you for these conditions.
Tell your provider if you have been drinking a lot of alcohol, more than 1 or 2 drinks a day.
If you smoke, try to stop. Ask your provider for help. Smoking can slow wound and bone
healing.
Tell your doctor about any cold, flu, fever, herpes breakout, or other illness you may have
before your surgery.
On the day of surgery:
Follow instructions about when to stop eating and drinking.
Take any drugs you're asked to take with a small sip of water.
Follow instructions on when to arrive at the hospital. Be sure to arrive on time
12. WHAT HAPPENS DURING SHOULDER ARTHROSCOPY SURGERY?
You will likely receive general anesthesia for this surgery. This means you will be asleep and unable to feel pain. Or, you
may have regional anesthesia. Your arm and shoulder area will be numbed, as a result you do not feel any pain. If you
receive regional anesthesia, you will also be given medicine to make you very sleepy during the operation.
During the procedure, the surgeon
Inserts the arthroscope into your shoulder through a small incision. The scope is connected to a video monitor in the
operating room.
Inspects all the tissues of your shoulder joint and the area above the joint. These tissues include the cartilage, bones,
tendons, and ligaments.
Repairs any damaged tissues. To do this, your surgeon makes 1 to 3 more small incisions and inserts other instruments
through them. A tear in a muscle, tendon, or cartilage is fixed. Any damaged tissue is removed.
At the end of the surgery, the incisions will be closed with stitches and covered with a dressing (bandage). Most surgeons
take pictures from the video monitor during the procedure to show you what they found and the repairs that were made.
Your surgeon may need to do open surgery if there is a lot of damage. Open surgery means you will have a large incision
so that the surgeon can get directly to your bones and tissues.
13. SHOULDER ARTHROSCOPY: RECOVERING AT HOME
Your surgery and recovery
Shoulder arthroscopy is a surgery to examine or repair tissues in or around your shoulder.
Because it is done with very small incisions, recovery is faster than with open surgery.
Still, your body will need time to recover.
For most people, this takes between 1 and 6 months, depending on what repairs were done.
There’s a lot you can do to make your recovery go better and help your shoulder heal
faster.
Managing pain
Managing your pain well is important. In addition to making you more comfortable, having
less pain will allow you to do the exercises you need to recover. Here are some ways to
manage pain:
Medicine. Your doctor will recommend a pain medicine for you. Be sure to take it right on
schedule at first. It’s easier to control the pain before it gets too bad. Your pain should
decrease after the first few days. If it doesn’t, be sure to tell your doctor.
Cold therapy, if your doctor recommends it. You may be instructed to put a cold pack over
your wound several times a day. This can help decrease pain and swelling.
14. CONT…
Caring for your incisions
Keeping your incisions clean and free of infection will help you heal faster and
prevent complications.
Protect your incisions - You will have a thick dressing on your shoulder. Leave
it in place until your healthcare providers say you can remove it. This may be a
few days after surgery
Keep your incisions dry - For the first few days, you’ll need to take a sponge
bath. You may not shower until after your dressing is removed. You may not
take a bath or go swimming until your wounds are healed, usually about 2 to 3
weeks after surgery. Fluid may leak from you incisions.
This is fluid that was used in your shoulder to help with the procedure, and is not a
problem. If your dressing gets very wet, though, ask your healthcare providers if it
should be replaced, or if you should place another dressing over the first one.
Your sutures (stitches) will be removed about 7 to 10 days after your surgery.
15. FAQS
What will rehabilitation involve?
Your healthcare providers will give you detailed information about your
recovery and rehabilitation exercises. In general, there are 4 goals to recovery
and rehabilitation.
These are:
Allow your shoulder to heal.
Regain your range motion.
Rebuild your strength.
Return to activities at the right time.
How can I prevent problems in the future?
The most important thing you can do is try to keep your shoulder strong and
flexible. Try to stay active. Ask your healthcare providers to recommend
exercises you can continue to do, and follow their instructions carefully.
16. CONT…
When should I see my doctor?
Your surgeon will need to check on your recovery and possibly remove
stitches or staples. Ask your surgeon how soon you should come back for a
follow-up visit. Schedule the appointment before you leave the hospital or
as soon as you get home, and be sure to keep it.
When should I call my doctor?
After surgery, it is normal to have some pain, tenderness, and stiffness. If
you have the following symptoms,
however, be sure to call your doctor:
Fever over 101°F after the second day following surgery
Increasing pain or swelling
Redness, warmth, or tenderness
Numbness or tingling in the arm or hand
