Procedures prior to dosing
The nurse/and or outreach worker must check that
the following are completed and included in the
patient’s medical record:
 Baseline registration form
 consent form
 doctor’s record
 medication sheet signed by the doctor

Prior to dispensing the medication the
nurse or outreach worker must:
 confirm the identity of the patient
 Confirm that the patient is not intoxicated
 Check the prescribed dose
 Confirm the dose again
 Record the dose in the daily medication
sheet

After recording dose details
1. Count and check the buprenorphine tablets into a mortar
and pestle
2. Crush the tablets into a fine powder
3. For new patients, issue the following instructions:
• place the powder under your tongue;
• do not chew;
• do not swallow saliva until powder has dissolved (3 - 5
minutes on average)

4. Place the powder onto a small sheet of paper and
tip the contents under the tongue
5. Observe the patient until you are satisfied the
powder has dissolved (usually > 2 minutes)
6. Patients should sign that they have received their
dose
