Evaluation of Community-based Cardiac Rehabilitation Program
in the Alberta Heartland Region

Alberta Heartland Primary Care Network
The AHPCN CR program was developed to meet the needs
of the AHPCN population by improving access to local
services and support that would contribute to secondary
prevention of heart disease. Cardiac rehabilitation has been
well demonstrated to slow or reduce total mortality and
morbidity and have a positive influence on participants’
quality of life.

Issue
The Alberta Heartland Primary Care Network (AHPCN)
requested data from the Northern Alberta Cardiac
Rehabilitation Program (NACRP) in 2012 which indicated
that from 2006-2010, 48% of all cardiac event patients
treated at either the Mazankowski Heart Institute or Royal
Alexandra Hospital live in rural areas. Only 3% of these
patients attended a Cardiac Rehabilitation (CR) program in
Edmonton. The main barriers to participation included cost,
time and distance.
The Program
The AHPCN CR program capacity has grown from 12
participants weekly in 2012 to 21 participants weekly in
2014. In 2015, the program will expand to meet demand and
will accommodate 30 participants weekly. The AHPCN CR
program consists of 12 weeks of supervised exercise and 7
educational classes provided by members of the AHPCN
multidisciplinary team.
The multidisciplinary team consists of:
•	Exercise Specialist
•	Registered Nurses
•	Registered Dietitians
•	Registered Psychologists
•	Pharmacist
•	Occupational Therapist
•	Social Worker
Referrals are accepted from:
•	Acute care hospitals located in Alberta
•	Northern Alberta Cardiac Rehabilitation Program
•	Alberta Heartland PCN physicians
