The most severely affected countries, Guinea, Sierra Leone and Liberia have very weak health systems, lacking human and infrastructural resources, having only recently emerged from long periods of conflict and instability. On August 8, the WHO Director-General declared this outbreak a Public Health Emergency of International Concern.
A separate, unrelated Ebola outbreak began in Boende, Equateur, an isolated part of the Democratic Republic of Congo.
The virus family Filoviridae includes 3 genera: Cuevavirus, Marburgvirus, and Ebolavirus. There are 5 species that have been identified: Zaire, Bundibugyo, Sudan, Reston and Taï Forest. The first 3, Bundibugyo ebolavirus, Zaire ebolavirus, and Sudan ebolavirus have been associated with large outbreaks in Africa. The virus causing the 2014 west African outbreak belongs to the Zaire species.

3. Nobel Prize in physiology
of .Medicine 2005
• 베리 마샬과 로빈워렌 박사는 위염과 소화
성궤양의 발생에 중요한 역할을 하는 헬리
코박터균을 발견하여 이 공로로 노벨상을
받음.
• 이 균의 발견으로 인해 소화성 궤양은 더 이
상 만성적이거나 불치의 병이 아니고, 완치
가 가능한 질환이 되었다.
4. • 오랫동안 궤양은 스트레스와 자극성의 음
식의 결과로 생긴 위의 병리적 형태로 여겨
져왔으나 베리 마샬과 로빈 워렌의 헬리코
박터 균 발견에 의해 위궤양은 세균 감염으
로 생기고 완전히 치유될 수 있음을 알게 되
었다.
• 이러한 사실은 처음 학회에 발표되었을때
지동설을 주장했던 갈릴레이 처럼 처음에
아무도 믿지 않았다.
5. • Barry Marshall and Robin Warren은 당시 팽배해
있고 누구도 부정하지 않았던 dogma인 소화성
궤양의 원인에 대하여,
• 세균 감염에 의하여 일어난다고 처음으로 주장하
였고 입증하였다.
• 이 발견으로 인하여 수천만명 이상의 사람이 소화
성 궤양을 항생제 치료로 완치할 수 있는 길을 열
어주었으며,
• 만성 세균 감염과 암의 관계에 대해 더 많이 이해
하게 되었다.
6. 발견
• 베리 마샬 박사는
워렌 박사의 발견에
흥미를 가지게 되었고
이 세균을 배양하기로
결정하였다.
이후 배양을 위해 많은 실험이 있었으나 배양되지 않다가 1982
년 부활절에 실수로 배양기에 plate 하나가 남겨졌고 이 plate내
에 세균 콜로니가 무수히 배양되었다.
• 곧 새로운 박테리아가 휴가기간이 끝나고 돌아온 마샬 박사에
게 발견되었고 새로운 박테리아는 모양에서 이름따
Helicobacter pylori로 명명 되었다.
7. • 웨렌 박사는 병리학자로서 위궤양을 앓은 환자의 위점막에 나
선형의 세균이 중요한 역할을 강한 위산 환경의 위점막에서 점
액으로 보호받고 있는 것을 발견하였다.
• 이 세균이 많은 점막부위에 위염과 위궤양이 심한 것을 관찰하
여 위염, 위궤양의 발생에 중요한 원인균이라는 것을 발견하게
되었다.
8. 일부의 감염자에게서 위장질환이
발생하는 것을 관찰할 수 있다.
헬리코박터 균 자체는 매우 변화가
많고 헬리코박터 균 자체의 유전적
변화는 위장질환의 위험에 다양한
영향을 준다.
또한 사람 개개인의 유전적 다양성
또한 질환의 발생에 영향을 주게
된다.
9. 헬리코박터 균 감염은 전세계 인구의 약 50%에서 감염되어 있고
나라마다 감염율이 차이가 많다.
대부분의 감염은 어린시절에 발생하며 엄마에게서 아이로 감염되게
된다. 일단 감염 되면 위장내에서 일생동안 남아 있게 된다.

• 마샬은 헬리코박터균 콜로니를 직접 마셨고 이후 극심한
위염과 위궤양에 시달렸고 결국 가설을 증명하게 되었다.

19. Relationship between
H.pylori & Human
• 현재에는 대부분의 소화성 궤양이 헬리코박터 균
감염에 의해서 발생하는 것으로 이해되고 있으
며, 대부분의 사람이 어린시절 감염된 이후 일생
동안 감염을 유지하게 된다.
• 전인류의 절반이상이 감염되어 있지만 운이 좋게
대부분의 감염자가 증상을 느끼지는 못한다.
20. • 헬리코박터 균은 오직 사람의 위장내에서
만 존재하며, 이러한 관점에서 볼때 헬리코
박터균과 숙주인 사람에게서 발생하는 위
궤양, 위암은 오랜시간동안 평화롭게 유지
되던 서로의 관계가 깨진 결과라고 볼 수 있
다.
21. • 이 균의 중요성을 인식하여 미국 NIH에서도 1994
년 2월 7∼9일에 Helicobacter pylori에 대한
Consensus Development Conference를 개최하
여 십이지장궤양이나 위궤양 환자에게 반드시 균
박멸치료를 하도록 공식적으로 천명하게되어 궤
양 치료에 역사적인 전환점을 마련하였다

22. 진단 방법
•
감염성 질환의 진단에서는 균배양이 가장 중요하지만 헬리코박터 균의 경우 예민도가 가장 떨
어지는 방법이다. (60-80%이내)
•
1) CLO test (rapid urease test) : 균의 urease에 의해 pH가 변하여
붉은색으로 양성 반응을 보임 (예민도와 특이도가 매우 높고
그자리에서 간편하게 결과를 알수 있다)
2) UBT test : (urea breath test)는 가장 좋은 비침습적 진단 방법으로
예민도와 특이도가 95% 정도로 매우높다.
3) 혈청 검사 - IgG가 가장 정확, 현재 만성적인 감염을 의미
내시경 없이 진단을 할 수 있는 좋은 검사방법
