2. Cancellation Policy: Conference enrollment may be cancelled through August 15, 2014, subject to a $200 US processing fee. No refunds or credits are
given for cancellations received after August 15, 2014. All no-shows are charged the full amount. All cancellations must be received in writing—mail, fax,
or e-mail is acceptable. Phone cancellations are not allowed. CSCMP membership fees are never refunded.

An attendee registering as a “CSCMP member” must maintain his or her ac-tive
CSCMP membership throughout the conclusion of CSCMP’s Annual Glob-al
Conference 2014. If your membership is due to expire before September
21, 2014, please check the “Lapsed Member” box on the other side of this
form to automatically renew after your expiration date.
Conference rates cover all program events—including meal functions— and
conference materials. They do not include hotel accommodations. Attend-ees
who have indicated they would like to renew their memberships or join
CSCMP on page one of this form will automatically have their membership
rates applied to their charges to cover the 12 months of CSCMP membership.
Membership belongs to the individual, not to his/her organization, and is
nontransferable and nonrefundable.
CSCMP assesses a $50 US processing fee for any conference or membership
payment that fails to clear the CSCMP bank and requests to change the form
of payment, such as from credit card to check.
Transfer Policy
If, after registering, you discover that you cannot attend the confer-ence
and must send someone in your place, you must inform CSCMP
in writing by mail, fax, or e-mail. Phone transfers are not allowed. The
transfer processing fee is $100 US. If you have registered for conference
as a CSCMP member and the person substituting for you is not already
a member, you must include a completed conference registration form,
plus $295 US to cover that person’s membership dues. CSCMP member-ships
are nontransferable and never refunded.
Please Note: Registration and attendance at, or participation in, the
CSCMP meetings and other activities constitutes an agreement by the
registrant to the CSCMP use and distribution (both now and in the future)
of the registrant or attendee’s image or voice in photographs, videotapes,
electronic reproductions, and audiotapes of such events and activities.
