Consultation Benefits: consultation* charge Rs. 500 you get on every time you visit for
any disease or opd treatment. There is no cap on the visit to the consultant.
Medicine Benefits: You will be entitled to prescribed medicine expense worth Rs. 2500
annually.

2. Root's Vatsal OPD Benefit Health Plan- Male Special
(Offer Price Rs. 5750 per member)
Consultation Benefits: consultation* charge Rs. 1000 you get on every time you visit for
any disease or opd treatment. There is no cap on the visit to the consultant.

any disease or opd treatment. There is no cap on the visit to the consultant.
Medicine Benefits: A Sum of ₹5000 for prescribed medicine expense.

Consultation Benefits: consultation* charge Rs. 500 you get on every time you visit for
any disease or opd treatment. Total consultations will be covered up to 12 which is a
maximum limit of the plan.
Medicine Benefits: You will be entitled to prescribed medicine expense worth Rs. 3000
annually.

6. Root's Gold OPD Health plan (Offer price Rs. 6399 onwards)
Consultation Benefits: consultation* charge Rs. 500 you get on every time you visit for
any disease or opd treatment. Total consultations will be covered up to 15 which is a
maximum limit of the plan.
Medicine Benefits: You will be entitled to prescribed medicine expense worth Rs. 4000
annually.
Plan Type: Individual / Family
Exclusion : All Fancy treatment including skin treatment , Dental, Ayurvedic, Homeopathy
and baba tantric type treatments.
How to claim the amount?

First of all, complete your opd treatment. After that the member/patient of above
mentioned OPD Benefit Health Plan must send his/her bills copy of OPD Treatment via
email or WhatsApp within 15 Days.
When and where will get the amount?
Your qualified amount will be credited to the registered user on paytm, Tej, Phone-pe or
NEFT to the bank within 10 days.
Can I transfer or donate my health plan to my friend or family member?
No, patient or user is not applicable to transfer this health plan to anyone.
Can I get benefit amount from my brother or friend's account?
No, Only registered user will get the amount.
Tax Saving u/s 80D
Who can claim Preventive Health Check Up deduction under Sec. 80D?
An individual or HUF can claim the deduction in the case of expenses on such health
check on family members as mentioned below.
