Virologists at Institute of Liver and Biliary Sciences (ILBS) have identified
`concurrent infection' in at least one of the 12 serum samples collected from
dengue patients this year. The patient was infected with more than one of
the four types of dengue virus. “It is a rare phenomenon and indicates the
endemic nature of the virus. Clinical manifestation of the infection in such
cases is very severe,“ said Dr Ekta Gupta, clinical virologist at the hospital.
Dengue fever is caused by four viral serotypes--type I that causes classic
dengue fever, type II that causes haemorrhagic fever with shock, type III that
causes fever without shock and type IV that causes fever without shock or
profound shock. Dr Gupta said they found type II and type III strains in one of
the samples tested at their institute. The first case of dual infection with two
dengue serotypes (dengue I and dengue IV) was reported in 1982, according
to an article published in Indian Journal of Medical Microbiology .

“Since then, more cases of concurrent infection by multi ple dengue serotypes have been
reported in different countries. In India, concurrent infection of an individual with more
than one dengue serotype is rarely reported,“ the article says.
AIIMS' virology laboratory of the department of microbiology, which is an apex laboratory
of National Vector Borne Disease Control Programme (NVBDCP) for dengue and
chikungunya, recently isolated dengue viruses in six out of nine samples, and identified the
type-II virus in four samples and type-IV in two other samples.
“Our report is based on preliminary data from the first lot of nine acute phase serum
samples of suspected cases which were inoculated into the C636 Aedes albopictus
(mosquito) cell line for virus isolation, followed by immunofluorescence using dengue type-
specific antibodies for detection and specification,“ said a senior AIIMS doctor. He said they
did not come across any case of concurrent infection.
But another doctor said, “We are seeing more frequent outbreaks and higher mortality due
to co-circulation of dengue serotypes. Concurrent infection is a matter of concern and it
could be behind the high mortality ,“ said a doctor.
