This is to signify our consent for our son/daughter, _______________________________________, to apply for
your Outbound Education®. We fully understand that in the event of my son/daughter’s participation in said trip, you will
take full responsibility for his/her safety and wellbeing, except only when any injury, damage or any untoward incident shall
occur due to the following:
1. Any fortuitous event such as typhoon, earthquake, floods, volcanic eruptions and other natural calamities and
events not within your control such as war, terroristic and violent attacks and similar occurrences;
2. Any pre-existing medical condition or disease;
3. Refusal to obey instructions, policies and guidelines set forth by World of Outbound, its officers and / or
employees.
It is also further understood that the officers and employees of World of Outbound shall exercise utmost diligence in
the charge of the person and property of our son/daughter during the course of the trip and that observance of the same shall
absolve the former of any liability arising from said trip.
