Improve accuracy of patient identification
Use at least two patient identifiers when providing care, treatment, or services.
o Identify individual as the person for whom service or treatment is intended
o Match service or treatment to that individual
Reduce risk of patient harm resulting from falls
Implement fall reduction program and include evaluation how effective it is
o Reduce risk of falling or injury
o Evaluation includes:
Fall history
Medication review
Walkers
