Virusul gripal




Virusul gripal (Myxovirus influenzae) apartine familiei Orthomyxoviridae. Are o forma
sferica, cu diametrul de 80-120 mm.

Virusul gripal contine un lant unic negativ ARN compus din
8 fragmente care codifica 10 proteine virale (materialul
genetic al virusului). Fragmentele de ARN au un invelis
proteic comun, care le uneste formand nucleoproteidul.
Invelisul exterior este constituit dintr-o membrana lipidica;
lipidele sunt raspunzatoare pentru complicatiile grave care
afecteaza          omul         in         timpul       bolii.



La suprafata virusului se afla structurile superficiale - hemaglutinina (denumita astfel
dupa capacitatea de a aglutina eritrocitele) si neuraminidaza (enzima).

Hemaglutinina asigura capacitatea virusului de a se uni cu celula receptor.

Neuraminidaza raspunde, in primul rand, de capacitatea particulei virale de a
patrunde in celula gazda si, in al doilea rand, de capacitatea particulelor virale de a
iesi din celula dupa multiplicare.

Nucleoproteidul (denumit si antigen-S) are structura specifica de tip; determina tipul
virusului (A, B sau C).

Structurile superficiale (hemaglutinina si neuraminidaza) au specificitate de subtip si
de tulpina; determina diferitele tulpini ale unui tip de virus. Exista 16 subtipuri
antigenice ale hemaglutininei (H1-H16) si 9 ale neuraminidazei (N1-N9). Aceste
antigene sunt supuse variatiei.

Exista 3 tipuri de virus gripal: A, B si C

Virusul gripal A produce imbolnavirea de gravitate medie sau mare. Infecteaza atat
omul cat si unele animale domestice (calul, porcul, pasarile). Virusul gripal A este
responsabil de aparitia pandemiilor si a epidemiilor extinse. Sunt cunoscute o
multitudine de subtipuri ale gripei A. Acestea se clasifica dupa antigenii superficiali
(hemaglutinina si neuraminidaza). In momentul de fata sunt cunoscute 16 tipuri de
hemaglutinina si 9 tipuri de neuraminidaza.

O caracteristica importanta a virusului este specificitatea, ce consta in faptul ca
virusul gripal nu poate avea 2 cauze diferite.

Ca si virusul gripal A, virusul gripal B este capabil sa isi modifice structura antigenica.
Gripa de tip B este prezenta numai la om si are manifestari epidemice moderate, cu
o evolutie lenta.

Virusul gripei C este destul de putin studiat. Este cunoscut ca, spre deosebire de
virusurile A si B, acesta contine doar 7 fragmente de acid ribonucleic si un antigen
superficial. Virusul gripal C infecteaza doar omul. Simptomele bolii sunt de obicei
foarte usoare sau nu se manifesta deloc. Acesta nu produce epidemii si nu duce la
urmari serioase. Este cauza unor imbolnaviri sporadice, in special la copii.

Structura antigenului nu este supusa unor astfel de transformari ca la virusul gripei
A. Imbolnavirile produse de virusul gripal C coincid deseori cu epidemia gripei de tip
A. Tabloul clinic este la fel ca si in cazul formelor usoare si moderat severe ale gripei
A.


Sistemul international de codare a virusurilor gripei

Nomenclatura virusurilor gripale foloseste un sistem international de codare. Aceasta
indica: tipul virusului, gazda de origine (pentru tulpini animale ale tipului A), originea
geografica, numarul tulpinii, anul izolarii, subtipul antigenic.

De exemplu: A/Bangkok/1/79(H3N2) este un virus de tip A, izolat in Bangkok in
1979.

Variatia antigenica este particularitatea fundamentala a virusurilor gripale A si B.
Aceasta are loc la nivelul antigenilor superficiali ai virusului (hemaglutinina si
neuraminidaza). Cel mai probabil este un mecanism evolutiv de adaptare a virusului
pentru asigurarea supravietuirii.
Exista doua mecanisme ale variatiei antigenice: minora (antigenic drift) si majora
(antigenic shift).
La virusul gripal A se intalnesc ambele variatii antigenice, in timp ce la virusul B se
manifesta numai variatia antigenica minora.

Variatia antigenica minora (antigenic drift) se produce in perioada interpandemica la
toate tipurile de virusuri. Acestea sunt mutatii de mica importanta in genele care
codifica hemaglutinina si neuraminidaza. De regula, astfel de transformari se produc
in fiecare an. Ca urmare apar epidemii; apararea data de contactele anterioare cu
virusul se mentine, dar este insuficienta.

Variatia antigenica majora (antigenic shift) La intervale neregulate de timp (10-40 de
ani) apar virusuri cu deosebiri mari fata de populatia de baza. Aceste modificari
afecteaza semnificativ structura antigenica a hematoglutininei si, mai rar, a
neuraminidazei. In prezent, mecanismul de formare a noilor tulpini ale virusului
gripal nu este in totalitate cunoscut. Una din teoriile existente se bazeaza pe
recombinarea genelor virusului gripal la animale (pasari, porci) si om. Atunci cand o
celula receptoare este infectata concomitent cu tulpini animale si umane, poate
produce un virus cu hemaglutinina sau neuraminidaza complet noua.

Ca urmare a variatiei antigenice majore (shift) se formeaza tulpini absolut noi de
virusuri, in fata carora marea majoritate a populatiei nu are imunitate. Aceste mutatii
sunt specifice doar virusurilor de tipul A si sunt responsabile de aparitia pandemiilor.
